Predicting the binding poses of ligands targeting RNAs is challenging. Here, we propose that using first-principles quantum mechanics/molecular mechanics (QM/MM) simulations, which incorporate automatically polarization effects, can help refine structural determinants ligand/RNA complexes in aqueous solution. In fact, recent advances massively parallel computer architectures (such as exascale machines), combined with power machine learning, are greatly expanding domain applicability these types notoriously expensive simulations. We corroborate this proposal by carrying out a QM/MM-based study on ligand CAG repeat-RNA, involved Huntington's disease. The calculations indeed show clear improvement properties, and they consistent NMR measurements, also performed here. Behavioral variant of Frontotemporal Dementia (bvFTD) and Bipolar Disorder (BD) share overlapping symptoms, complicating diagnosis. BvFTD, especially linked to C9orf72 expansions, often mimics BD, highlighting the need for reliable biomarkers. This study aimed differentiate bvFTD from BD using miRNA profiles in neural-enriched extracellular vesicles (NEVs). A cohort 100 subjects was analyzed: 40 (20 sporadic, 20 carriers), healthy controls. NEVs were isolated plasma profiled real-time PCR. Among 754 miRNAs, 11 significantly deregulated BD. MiR-152-5p downregulated sporadic bvFTD, while let-7b, let-7e, miR-18b, miR-142-5p altered carriers. patients showed distinct patterns miR-331-5p, miR-335, miR-345 compared bvFTD. Abstract Multiple sclerosis (MS) and Neuromyelitis Optica (NMO) are immune-related CNS inflammatory diseases that often present with overlapping clinical symptoms, leading to frequent misdiagnosis, particularly in aquaporin-4 seronegative NMO patients. Identifying the underlying mechanisms of these is critical for discovering biomarkers enable timely diagnosis effective treatment. This study included 252 participants, divided into four groups. Group I (Relapsing–Remitting MS: RRMS group), II (Secondary Progressive SPMS III (NMO IV (Healthy controls). Blood samples were collected from all participants measure expression levels Neuregulin-1 (NRG1), lncRNA Ftx, miR-382-5p .
