ABSTRACT Objective To analyse global prevalence data for infertility due to endometriosis from 1990 2021, emphasising health inequalities. Design Population‐based study. Setting Data the Global Burden of Disease (GBD) database. Population Individuals diagnosed with endometriosis. Methods A statistical method was employed evaluate changes in disease over time. We also analysed how varies by age, time period and birth cohort. model used predict future trends. Additionally, we examined relationship between socio‐demographic index (SDI) levels across countries. Finally, conducted a decomposition analysis identify key factors driving assessed inequality. Main Outcome Measures The burden Results 2021 showed downward trend, low SDI region had notably higher burden. High risk observed 25–29 age group effects analysis. Period risks almost kept decreasing these years, cohort effects, later born individuals an overall lower than earlier individuals. Endometriosis (EMs) is a prevalent chronic gynecological condition that depends on estrogen, marked by the presence of active endometrial tissue (glands and stroma) outside uterus. Although pathologically benign, it exhibits biological behaviors such as invasion metastasis akin to malignant tumors. Endometriosis-associated ovarian carcinoma (EAOC), arising from transformation EMs, poses significant clinical challenges. However, mechanisms underlying EAOC pathogenesis remain incompletely understood, with lack reliable biomarkers for early diagnosis personalized treatment strategies. Considering number EMs patients extended period during which can occur, deserves attention. Current research both domestically internationally indicates complex, involving genetic mutations, immune microenvironment, oxidative stress, epigenetic changes, related areas.
