The endoplasmic reticulum (ER) is crucial for protein quality control, and disruptions in its function can lead to various diseases. ER stress triggers an adaptive response called the unfolded (UPR), which either restore cellular homeostasis or induce cell death. Melatonin, a safe multifunctional compound, shows promise controlling could be valuable therapeutic agent managing UPR. By regulating mitochondrial functions, melatonin helps maintain via reduction of oxidative stress, inflammation, apoptosis. Melatonin directly indirectly interfere with ER-associated sensors downstream targets UPR, impacting death, autophagy, molecular repair, among others. Crucially, this review explores mechanistic role on diseases including liver damage, neurodegeneration, reproductive disorders, pulmonary disease, cardiomyopathy, insulin resistance, renal dysfunction, cancer. Melatonin, N-acetyl-5-methoxytryptamine, is a neuroendocrine hormone secreted by the pineal gland. This pleiotropic indoleamine possesses amphiphilic properties, allowing it to penetrate most biological barriers and exert its effects at subcellular level. ABSTRACT Melatonin exhibits various biological functions, including regulation of circadian and endocrine rhythms, anti‐inflammatory, antioxidant effects. Aging damaged mitochondria are major sources oxidative stress (OS), mitochondrial quality control (MQC) is crucial for maintaining normal function. Delaying the development of atherosclerosis (AS) and decreasing cardiac ischemia-reperfusion damage remain serious challenges for medical community. Chronic arterial disease, i.e., AS, is frequently linked to oxidative stress inflammation as significant contributing causes. AS risk factors, such hyperlipidemia, high blood pressure, age, hyperglycemia, smoking, cholesterol, irregular sleep patterns, can exacerbate in carotid artery further shrink its lumen. Finding new approaches that support plaque inhibition or stability an ongoing problem. The last ten years have shown us melatonin (MLT) affects cardiovascular system, although exact mechanisms action are yet unknown. Atherosclerosis is the underlying factor in premature death of millions humans annually. In recent years, there has been an interest in studying the specifics of relationships between metabolic syndrome (MetS), sleep disorders and melatonin (MEL). Dyssomnia insomnia are important risk factors for insulin resistance, obesity, MetS, type 2 diabetes mellitus with a degree influence similar to traditional factors. Thus, treatment dyssomnia is one key tasks prevention MetS. The regulation circadian rhythms by exogenous intervention (in particular, use melatonergic drugs) likely play central role MEL acts as powerful antioxidant that can cross blood-brain barrier, suppress oxidative stress, chronic inflammation, interact gut microbiome.
