Abstract Risk prediction in emergency medicine (EM) holds unique challenges due to issues surrounding urgency, blurry research-practise distinctions, and the high-pressure environment departments (ED). Artificial intelligence (AI) risk tools have been developed with aim of streamlining triaging processes mitigating perennial affecting EDs globally, such as overcrowding delays. The implementation these is complicated by potential risks associated over-triage under-triage, untraceable false positives, well for biases healthcare professionals toward technology leading incorrect usage tools. This paper explores an analysis a case study involving machine learning triage tool called Score Emergency Prediction (SERP) Singapore. used estimating mortality presentation at ED. After two successful retrospective studies demonstrating SERP’s strong predictive accuracy, researchers decided that pre-implementation randomised controlled trial (RCT) would not be feasible how interacts clinical judgement, complicating blinded arm trial. led them consider other methods testing real-world capabilities, ongoing-evaluation type studies. We discuss outcomes risk–benefit argue proposed strategy ethically appropriate aligns improvement-focused systemic approaches implementation, especially health systems framework (LHS) ensure safety, efficacy, ongoing learning.
