Besides the diagnostic use for infectious diseases in a point-of-care clinical settings, simplicity and ease of self-performed RAT can also be an alternative approach informing disease surveillance at community level, carrying potential advantage enhanced timeliness, acceptability, flexibility. Commissioned by Hong Kong Government during catastrophic fifth wave COVID-19 pandemic, our team established maintained ad-hoc large-scale participatory daily antigen rapid testing (DARTS) system real-time situational awareness SARS-CoV-2 activity to inform policy consideration timely manner. This Chapter will describe concept design approach, examine practical feasibility challenges, related logistical on implementation maintenance, technical aspects data analysis cater unique need, other additional contribution understanding novel (estimating vaccine effectiveness, symptomatology viral shedding pattern).

Abstract The Omicron variant of concern has a high level mutations in different genes that raised awareness about the performance immunological products such as vaccines and antigen detection kits. In this systematic review meta‐analysis, we investigated whether had significant influence on rapid test (RAT) comparison to PCR. We registered meta‐analysis PROSPERO with registration number CRD42022355510. searched PubMed, Scopus, Embase, Web Science databases systematically 1 August 2022. After article screening, assessed quality included studies based JBI checklist. Following data extraction, performed using R software. 18 qualified articles presenting sufficient RATs RT‐PCR infections. pooled specificity sensitivity were 1.000 (0.997–1.000) 0.671 (0.595–0.721), respectively. FDA‐approved kits showed better than WHO‐approved ones 0.728 (0.620–0.815). use nasal swabs higher compared nasopharyngeal swabs. for samples CT‐value >25 was 0.108 (0.048–0.227). Rapid tests show impaired COVID‐19 diagnosis when is circulating, particularly low viral loads.

Abstract Since late 2021, the variant landscape of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been dominated by concern (VoC) Omicron and its sublineages. We others have shown that detection Omicron-BA.1 -BA.2-positive specimens rapid antigen tests (RATs) is impaired compared to Delta VoC-containing samples. Here, in a single-center retrospective laboratory study, we evaluated performance ten most commonly used RATs for Omicron-BA.4 -BA.5 infections. 171 swab from SARS-CoV-2 RNA-positive patients, which 71 were classified as BA.4 100 BA.5. All swabs collected between July September 2022. 50 PCR-negative samples healthy individuals, October 2022, showed high specificity 9 out 10 RATs. When assessing analytical sensitivity using clinical specimens, 50% limit (LoD50) ranged 7.6 × 4 3.3 6 RNA copies subjected 6.8 3.0 Overall, intra-assay differences these two subvariants not significant both tissue culture-expanded virus isolates. In contrast, marked heterogeneity was observed among RATs: be positive point-of-care tests, up 443-fold (BA.4) 56-fold (BA.5) higher viral loads required worst performing RAT best RAT. True-positive rates Omicron-BA.4- or -BA.5-containing highest load category ( C t values < 25) 94.3 34.3%, dropping 25.6 0% with intermediate (25–30). conclude remains challenge general public obtain reliable results evolving subvariant-driven pandemic.

During 2022, the COVID-19 pandemic has been dominated by variant of concern (VoC) Omicron (B.1.1.529) and its rapidly emerging subvariants, including Omicron-BA.1 -BA.2. Rapid antigen tests (RATs) are part national testing strategies to identify SARS-CoV-2 infections on site in a community setting or support layman's diagnostics at home. We others have recently demonstrated an impaired RAT detection caused compared Delta. Here, we evaluated performance five RATs single-centre laboratory study examining total 140 PCR-positive respiratory swab samples, 70 Omicron-BA.2, as well 52 PCR-negative swabs collected from March 8th until April 10th, 2022. One test did not meet minimal criteria for specificity. In assessment analytical sensitivity clinical specimen, 50% limit (LoD50) ranged 4.2 × 10

The new severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus has high infectivity, often masked by asymptomatic carriers, which allows it to spread rapidly and become a pandemic. Attempts slow the pandemic at this stage depend on ability unmask carriers. rapid diagnosis of active disease 2019 (COVID-19) infection is one cornerstones control, as nasal cavity main gateway for SARS-CoV-2 entry altered sense smell feature current virus. In present study, we therefore tested olfactory threshold coupled with heart-lung parameters in subjects undergoing traditional molecular testing, resulting significantly different score between healthy controls. total, 82% positives showed impairment; these, 46% had hyposmia 7% anosmia, while control 9% 0% respectively, agrees heart rate, breathing blood pressure parameter variations. test physiological may help identify people. conclusion, our results suggest that most individuals could be unmasked mass screening then referred slower diagnostic tests.

However, RATs preferred for (59.1%) over (51.1%). 24.0% all individuals willing to ignore hygiene measures 76.9% less afraid transmission (5.9% 48.8% case RAT). 28.8% reported not self-isolate RAT. Multivariate analyses revealed that non-vaccinated relied governmental (p = 0.0004) more social 0.0216), disbelieved ≤ 0.0001) while displaying strong preference towards using 0.0001), abandon pandemic-related transmitting RAT likely isolate 0.0001).Insights into sources perception, preferences, related are key refining health surveillance campaigns. Background Molecular tests can detect lower concentrations of viral genetic material over a longer period respiratory infection than antigen tests. Delays associated with central laboratory testing result in hospital-acquired transmission, avoidable patient admission, and unnecessary use antimicrobials, all which may lead to increased cost management. The aim this study was summarize comparisons clinical outcomes rapid molecular diagnostic (RMDTs) versus other for infections. Methods A systematic literature review (SLR) conducted April 2023 identified studies evaluating patients suspected having Results SLR included 21 studies, seven 14 compared RMDTs (conducted at points care or laboratories) standard (non-rapid) SARS-CoV-2 influenza, respectively. In SARS-CoV-2, led reductions time test results (range the reported medians: 0.2–3.8 hours 4.3–35.9 hours), similar length emergency department stay (3.2–8 3.7–28.8 hours). Similarly, (1–3.5 18.2–29.2 (3.7–11 3.8–11.9 were found decrease exposure uninfected patients, rate hospitalization, hospitals, frequency antiviral antibacterial therapy, while improving flow, Conclusions Compared tests, improve outcomes, turnaround time, stewardship by decreasing antibiotics antivirals.
