Background Emerging observational studies indicated an association between hyperthyroidism and gastrointestinal disorders. However, it remains unclear whether this is causal, particularly in the case of gastroesophageal reflux (GERD) esophageal cancer. Methods To assess potential causal relationship GERD or cancer, we conducted a bidirectional 2-sample Mendelian randomization study. Independent genetic instruments for from UK Biobank (N case=3,545 N control=459,388) public genome-wide study (GWAS) dataset case=3,731 control=480,867) were used to investigate with cancer case=740 control=372,016) GWAS database case=20,381 control=464,217). Four different approaches (inverse variance weighted (IVW), mode, MR-Egger, median regression) ensure that our results more reliable. Additional sensitivity analyses also performed validate results. Results When was considered as exposure factor, appeared act protective factor (OR IVW = 0.88, 95% CI, 0.79-0.99, P 0.039), while risk 1.03, 1.01-1.06, 0.003). there no evidence supporting reverse susceptibility Conclusion Our findings provided relationships These substantiate certain discoveries previous on level provide insight into relevant factors.

Increased interest in the pathogenesis and evolution of autoimmune chronic atrophic gastritis (A-CAG) has led to search for serological markers that can be used detect changes gastric mucosa at an early stage monitor course disease. Parietal cell autoantibodies have been proposed as suitable immunological damage, they detected serum when symptoms are not yet present. However, utility measuring only level parietal follow-up A-CAG does appear suffice. Recent evidence suggested that, monitoring A-CAG, antibodies should associated with evaluation function through biochemical hormonal tests, such pepsinogens gastrin 17. This integrated approach will allow more effective real-time state mucosa. As is a progressive disorder increased risk cancer neuroendocrine tumors, precise patients atrophy needs better defined. Further longitudinal studies large cohorts must performed long-term follow-up.

There is a growing interest in autoimmune gastritis (AIG), particularly regarding its prevalence and natural history. Retrospective observational study including all patients with positive parietal cell antibodies performance of gastroscopy between 2013 2023. The first subsequent was defined as follow-up endoscopy considered for histological comparison. Categorization stages made into stage 0 (potential), I (non-atrophic gastritis), II (atrophic gastritis) III (dysplasia, neuroendocrine tumor or adenocarcinoma). total 426 were included, 316 females median 54.4 years (IQR 45.3-63.2). During this period, 26798 underwent at least one upper endoscopy, so the AIG 1.6% (95% CI 1.4-1.7%). Histologically, 105 classified potential (24.7%), 99 (23,2%), 215 (50.5%) 7 (1.6%). 15364 presented follow endoscopy. A significant increase advanced observed (difference 18.72.1% 95% 7.61.4%-292.6%; p=0.0013) decrease GAI -20.214% 11.8-5.5% -28.722.5%; p<0.001). At baseline 39 exhibited hyperplastic polyps (9.2%), 8 fundic gland (1.9%), 3 adenomas low-grade dysplasia (0.7%) G1 tumours (0.7%). Only patient (0.2%) diagnosed signet-ring gastric carcinoma. low among undergoing gastroscopy. Biopsies revealed that around half population atrophy notable progressive disease.

ABSTRACT Background Despite the low prevalence of Helicobacter pylori in Indonesia, high incidence gastritis, predominantly atrophic suggests that factors such as autoimmune gastritis (AIG) contribute to this unusual pattern. This study aims investigate epidemiology AIG, histopathology, and its association with H. status Indonesia. Methods A cross‐sectional was conducted various regions Indonesia between 2014 2017; 380 eligible sera gastric biopsies were available when conducted. As many 138 samples included based on initial examination by updated Sydney system. The diagnosis AIG confirmed serologic testing for parietal‐cell antibodies (PCA) detailed histopathological assessment sparing antrum features. Results Among study, 78.99% (109/138) PCA positive (≥ 10 RU/mL) 0.72% (1/138) considered be diagnosed (spared from features). majority cases (61/109; 55.96%) a significant correlation ( p < 0.05, R = 0.31). Additionally, found infection level features 0.05). Conclusion demonstrates without is not attributable only single AIG‐positive case found. These findings underscore important role pathogenic factor chronic highlight mechanisms triggering immune responses driving disease progression changes.

Parietal cell autoantibodies (PCAs), which recognize the enzyme H+/K+-ATPase as a target, are considered to be diagnostic marker of autoimmune gastritis and pernicious anemia; these conditions characterized by presence corpus atrophic gastritis. Circulating PCAs can detected using several analytical methods that commonly available in clinical laboratory. Traditionally, indirect immunofluorescence (IIF) on rodent or primate stomach tissue is used screening test for detection PCAs. However, IIF suffers from high inter-observer variability lacks standardization. In addition, like immunoblotting, results expressed only qualitative semi-quantitative manner. Based few studies reviewed herein, quantitative enzyme-linked immunosorbent assays (ELISAs) fluorescence immunoassays (FEIAs) purified perform better than PCAs, displaying higher sensitivity utility monitoring disease. light their accuracy, solid-phase should preferred The use detect antibodies versus specific subunit (α β) currently confined world research. The predominant characteristic of autoimmune gastritis (AIG) is corpus-dominant advanced atrophy, which mostly observed in the middle to late stages. More reports are needed on endoscopic features early stage. In this report, we present two cases early-stage AIG examinations showed no atrophy gastric mucosa but displayed a transition collecting venules from regular an irregular arrangement. addition, yellowish-white cobblestone-like elevations were fundic gland region. Histologically, manifestations included pseudohypertrophy and protrusion parietal cells into lumen, possibly along with hyperplasia G cells, lymphocytic infiltration potentially pseudopyloric metaplasia. Serologically, anti-parietal cell antibody returned positive results, whereas anti-intrinsic factor yielded negative results. study, summarized some patients, aiming provide clues for endoscopists detect AIG.

Guillain-Barré syndrome (GBS) and autoimmune gastritis (AIG) are both diseases (ADs) that have a low prevalence in China. Both conditions involve the immune system mistakenly attacking body's own tissues. GBS primarily affects peripheral nervous system, leading to muscle weakness paralysis, while AIG targets stomach lining, causing inflammation reduced absorption of vital nutrients. Subacute combined degeneration (SCD) spinal cord is most common neurological manifestation vitamin B12 deficiency. As yet, there been no reported cases patients with complications including SCD. We report case 54-year-old male patient who had experiencing progressive numbness his extremities, burning tingling sensations, cotton-stepping sensation, difficulty walking for three weeks. He was admitted hospital underwent an extensive medical workup. Magnetic resonance imaging (MRI) cervical spine showed abnormal signal intensity consistent typical manifestations Gastric endoscopy revealed local atrophy gastric corpus, tissue biopsy indicated atrophic intestinal metaplasia, diagnosis AIG. Lumbar puncture cerebrospinal fluid (CSF) results albumincytological dissociation, further confirming GBS. treated intravenous immunoglobulin methylcobalamin therapy these significant clinical improvement upon discharge.
