В обзоре представлены основные сведения об ультрафиолетовом кросслинкинге (сшивании) роговицы, который нашел широкое применение в офтальмологии в лечении различных видов кератэктазий. Кросслинкинг основан на ультрафиолетовом (УФ) облучении роговицы длиной волны 370 нм в присутствии рибофлавина, приводящем к интракорнеальным фотохимическим взаимодействиям. В результате процедуры сшивания коллагена наблюдается увеличение прочностно-механических свойств роговицы, приостанавливается прогрессирование заболевания. В статье отображены этапы разработки метода и способы его выполнения, сферы клинического применения, описаны особенности происходящих биомеханических, биохимических, морфологических и ультраструктурных изменений в роговице, вызванных рибофлавин-УФА-индуцированным сшиванием.

Ultraviolet cross-linking (UVCL) of corneal collagen is one the promising methods for treating bacterial ulcers. Purpose work: to evaluate effectiveness and safety use a new domestic device UVCL KERATOLINK in treatment patients with ulcer. Material methods. Local was performed 24 (24 eyes) aged from 18 74 years, who made up main group. The comparison group included 10 same age To treat ulcerative lesions, drug therapy (antibacterial, corticosteroid, tear replacement reparative) used both groups, but also procedure 5 minutes using “accelerated cross-linking” program. Each patient underwent 3 procedures an interval 2 days. Ophthalmological examination before treatment, days after 1st, 2nd 3rd procedures, as well 1, 3, 6 months completion visometry, tonometry, microbiological discharge, determination size defect, optical coherence tomography anterior department eye (OCT-AS). examined at time set Results. All were completed without complications, no adverse events identified, all cases recovery period uneventful. UVCL, average uncorrected visual acuity increased 0.029±0.026 0.195±0.175. It has recently been proposed to use ultraviolet (UVA) corneal collagen crosslinking as an alternative method of treating bacterial ulcerative lesions the cornea. Ambivalent results regarding efficacy and safety this treatment limit its implementation in clinical practice so far. The purpose work is analyze three cases involving a new device for local UVA complex protracted-course ulcers with suspected development mixed fungal infection. Material methods. For dynamic follow-up patients, we used biomicroscopy fluorescein test, photo registration optical coherence tomography anterior part eye. Inflammatory changes were assessed using point scale. Results. 3–5 procedures exposure (with instillations 0.1% riboflavin solution) lasting minutes each given interval 2–4 days ensured stable fast epithelialization cornea significant increase visual acuity inflammatory process outcome. Purpose : to compare the effectiveness of corneal ulcer treatment by antibiotic therapy and local ultraviolet crosslinking in a clinical morphological experimental study using new Crosscor device. Material methods . The was performed on 15 rabbits (30 eyes) with model ulceration caused Staphylococcus aureus. Rabbits were divided into 3 groups 5 specimens each (10 eyes). Group 1 received A range (UVA) (CL) cornea device (3 procedures lasting 5, 6 minutes given intervals three days). In group 2, CL combined antibacterial (AB): instillations tobramycin 0.3 % ofloxacin ointment mg / g 4 times day for 14 days. control group, only AB given. dynamics inflammation, intensity turbidity size evaluated biomicroscopy, fluorescein test, photoregistration anterior part eye, according point system. Morphological changes determined light microscopy (Leica DFC 420C digital camera). Objective of the study. To analyze effectiveness accelerated local corneal cross linking in treatment keratoconus (KC). Patients and methods. Thirty-four patients (42 eyes) were included study: 19 people (24 with stage II keratoconus, 15 (18 III Amsler keratoconus. Purpose . To study the effects of corneal collagen cross-linking (CCL) using ultraviolet-A (UVA) irradiation with riboflavin on eye’s anterior segment structures in ulcers and other keratopathies animals. Material methods. The involved 26 animals (39 eyes) — 10 cats, dogs, 6 horses ulcers, staphyloma, dry autoimmune keratoconjunctivitis. Examination included slit-lamp biomicroscopy, ophthalmoscopy (in cases transparent cornea), tear production test, staining eye surface vital dyes. CCL was performed UVA radiation LEDs a fiber optic output (wavelength 370 nm, power 3 mW/cm2) combined 0.1 % instillation according to protocol we developed, which specifies various numbers durations depending diagnosis therapeutic effect desired. Results. Restoration shape transparency cornea, relief inflammation, normalization occurred much sooner than traditional treatment. restoration vision 2 months earlier eyes staphyloma; more 4 5 keratoconjunctivitis; no relapses were noted ulcerative processes, function stratified squamous epithelium achieved within or weeks. Ultraviolet cross-linking (UVCL) of corneal collagen is one the leading methods for treating primary progressive keratectasia. Purpose : to evaluate effectiveness and safety a new Russian-produced device UVCL, KERATOLINK, in patients with stage I–II keratoconus pellucid marginal degeneration (PMCD). Material . UVCL was performed 36 (46 eyes) aged 18 35 years, including 22 (30 14 (16 PMCD. Three programs were used: 1 — standard radiation intensity from 2.5 3.0 mW/cm2 30-minute exposure time (8 patients, 10 eyes); 2 accelerated 1: 8.0 9.0 min (20 25 3 16.0 18.0 5-minute 11 eyes).
