Abstract Background Diagnosis in psychiatry faces familiar challenges. Validity and utility remain elusive, confusion regarding the fluid arbitrary border between mental health illness is increasing. The mainstream strategy has been conservative iterative, retaining current nosology until something better emerges. However, this led to stagnation. New conceptual frameworks are urgently required catalyze a genuine paradigm shift. Methods We outline candidate strategies that could pave way for such These include Research Domain Criteria (RDoC), Hierarchical Taxonomy of Psychopathology (HiTOP), Clinical Staging, which all promote blend dimensional categorical approaches. Results alternative still heuristic transdiagnostic models provide varying levels clinical research utility. RDoC was intended framework reorient beyond constraints DSM. HiTOP began as derived from statistical methods now pursuing Staging aims both expand scope refine diagnosis by inclusion dimension timing. None yet fit purpose. Importance Over the past decade, leadership of National Institute Mental Health (NIMH) has emphasized importance a transdiagnostic approach to psychiatric investigation using Research Domain Criteria (RDoC) mapping more closely neurobiology. Objective To investigate whether research support from NIMH for individual RDoC domains and changed over time had differential impact in terms publication, citations, or patent filings. Design, Setting, Participants In this longitudinal cohort study, all R01, R21, R03 studies funded by between January 2003 December 2023 were identified via Institutes RePORTER database. Their abstracts characterized (negative valence, positive cognition, social, arousal, sensorimotor) large language model. Main Outcomes Measures Primary outcomes publications, citation estimated at 5 years index year funding, patents, examined regression models adjusted other grant characteristics. Results Among 8897 R03, R21 projects, reflecting $17.7 billion investment, 3141 (35.3%) reflected negative valence; 1344 (15.1%), 2781 (31.3%), cognition; 1607 (18.1%), social; 343 (3.9%), arousal; 571 (6.4%), sensorimotor domains. A total 1793 (20.2%) incorporated perspective. Positive social associated with fewer publications (difference, −1.13 [95% CI, −2.11 −0.15] −2.23 −3.15 −1.30], respectively) lesser −0.47 −0.75 −0.18] −1.19 −1.46 −0.91], years. Social (adjusted odds ratio [AOR], 0.11; 95% 0.04-0.23) cognitive (AOR, 0.66; 0.48-0.89) proposals 0.37; 0.21-0.60) lower likelihood filing. Conclusions Relevance study grants different differed substantially their scientific generation. Background The field of biomedical research is entering a new era, in which public data sharing increasingly the norm. There are many advantages embracing initiatives, including tackling replication crisis through enhanced transparency and publication null findings, facilitating global collaborations to accelerate progress, enhancing cost-effectiveness by reducing duplication efforts, making scientific advances more accessible public. However, there also several crucial ethical logistical challenges that must be addressed maximise benefits minimise risks. potential, recognised, risks unregulated (e.g., reidentification, misuse, lack representativeness due variability who agrees share data) have been exemplified high profile breaches directly clash with efforts make robust, accessible, global. Methods/Results Here, we narratively outline current for from perspective child adolescent psychiatry, one area where they may particularly acute. For example, early often requires caregivers consent on behalf minor – increasing responsibility researchers consider how science today evolve into future (when those individuals no longer minors).
