Sentinel event review systems were created to prevent catastrophes from occurring in complex human organizations, though they have yet be fully realized corrections. reviews may particularly salient for incarcerated people who engage dual harming behaviors (i.e., self-harm and violence) given their overrepresentation disciplinary incidents. To explore this potential, incident reports spanning a three-year period an entire state prison system examined. Our specific objectives assess the prevalence of institutional incidents committed by harm, identify unique characteristics. Quantitative estimations indicated how harmers, comparison other population members, responsible higher proportion Further analyses harm disproportionately more at-risk mental health issues, likely housed maximum-security facilities. Policy implications are discussed.

Background Co-occurring self-harm and aggression (dual harm) is particularly prevalent among forensic mental health service (FMHS) patients. There limited understanding of why this population engages in dual harm. Aims This work aims to explore FMHS patients’ experiences harm how they make sense behaviour, with a focus on the role emotions. Method Participants were identified from their participation previous study. Sixteen patients lifetime history recruited two hospitals. Individuals participated one-to-one, semi-structured interviews where reflected past and/or current aggression. Interview transcripts analysed using reflexive thematic analysis. Results Six themes generated: as emotional regulation strategies, consequences witnessing harmful behaviours, relationships others self, trapped within criminal justice system, convergence divergence aggression, moving forward an patient. Themes highlighted shared risk factors across participants, including dysregulation, perceived lack social support behaviours. underlined duality primarily utilising both regulate negative These behaviours also fulfilled distinct purposes at times (e.g. punishment, defence). The impact contextual FMHSs, restrictive practices institutionalisation, emphasised. Conclusions Findings provide recommendations that can help address settings, (a) transdiagnostic, individualised approaches consider aggression; (b) cultural organisational recovery-centred practice.

Abstract Background Dual harm encompasses the complex interplay of co-occurrence self-harm and aggression. Individuals with dual may display a more hazardous pattern harmful behaviors like homicide-suicide compared to people sole harm. This study aimed examine presence among general psychiatry inpatients in mental health unit Uganda. Methods A retrospective chart review 3098 from January 2018 December 2021. reported experience at admission was based on experiences or property both. Logistic regression assessed association between sociodemographics clinical characteristics. Results total 29 (1%) patients experienced harm, five having both others property, 23 people, one property. statistically significantly associated male gender bivariate analysis. However, there were no significant factors multivariate analysis sensitivity specific types Conclusion General Uganda before lower prevalence than previous literature. investigated sociodemographic could explain these experiences. Further studies looking are warranted understand unfortunate serious consequences

The aetiology of dual harm (co-occurring self-harm and violence towards others) is poorly understood because most studies have investigated separately. We aimed to examine childhood risk factors for self-harm, violence, harm, including the transition from engaging in single harm.Data Avon Longitudinal Study Parents Children, a UK-based birth cohort study, were used estimate prevalence self-reported engagement at ages 16 22 years. Risk ratios calculated indicate associations across various risks age years 22.At years, 18.1% 4176 members had harmed themselves, 21.1% engaged others 3.7% reported harm. At equivalent estimates increased 24.2, 25.8 6.8%, respectively. Depression other mental health difficulties, drug alcohol use, witnessing being victim of, or witnessing, associated with higher transitioning by 22.Prevalence doubled highlighting importance early identification intervention during this high-risk period. Several psychosocial specifically been identified.

Dual harm is the co-occurrence of self-harm and aggression during an individual's lifetime. This behaviour especially prevalent within criminal justice forensic settings. The forms that should be included in definition dual have not yet been established. study aimed to use network analysis inform evidence-based by assessing relationship between different aggressive young people (N = 3,579). We used data from Avon Longitudinal Study Parents Children (ALSPAC). Results revealed low correlations variables, leading sparse models with weak connections. found when separated into their distinct forms, acts are only weakly correlated. Our work provides preliminary evidence assist understanding managing clinical settings informs recommendations for future research.

Self- and other-oriented harmful behaviors are common among emerging adults. Individuals who engage in both forms of behavior, termed dual-harm, experience more adverse outcomes comparison to individuals either. This study examines temperamental traits, defined as reactive regulative temperament, transdiagnostic factors underlying engagement self-oriented, other-oriented, dual-harmful behaviors. These operationalized non-suicidal self-injury (NSSI), direct aggression, a combination both, respectively. Participants 669 adults (69.36% women; Mage = 21.48; SD 2.20). The focusses on how temperament can differentiate four different groups (i.e., no-harm, NSSI-only, aggression-only, dual-harm group) pairwise manner. Results highlight hyper-reactive Behavioral inhibitions system (BIS) hypo-reactive activation (BAS)-Impulsivity compared no-harm aggression-only. Conversely, we found BIS BAS-Impulsivity those report aggressive behaviors, NSSI-only. Finally, Effortful control (EC) is the group, when engaging only NSSI, or neither highlights role traits differentiating patterns which should be integrated future research clinical interventions.
