Obesity has emerged as a global epidemic with far-reaching health complications, including its role an independent risk factor for chronic kidney disease (CKD). Increasing evidence suggests that obesity contributes to CKD through multiple mechanisms, inflammation, hemodynamic alterations, insulin resistance, and lipid accumulation. These processes can culminate in histopathological changes collectively referred obesity-related glomerulopathy (ORG). This review aims provide comprehensive overview of the current knowledge regarding prevalence, clinical manifestations, pathophysiology ORG. Furthermore, we emphasize importance identifying key biomarkers facilitate early detection Finally, explore emerging therapeutic strategies offer promise mitigating this growing crisis.

Abstract Purpose of Review Choline is an essential nutrient for human health and cellular homeostasis as it necessary the synthesis lipid cell membranes, lipoproteins, neurotransmitter acetylcholine. The aim this review to analyze beneficial effects choline its significance in metabolism various inflammatory pathways, such inflammasome. We will discuss dietary cardiometabolic disorders, non-alcoholic fatty liver disease (NAFLD), cardiovascular (CVD), chronic kidney (CKD) well cognitive function associated neuropsychiatric disorders. Recent Findings deficiency has been related development NAFLD disability offspring adulthood. In sharp contrast, excess intake mediated via increased production trimethylamine by gut microbiota trimethylamine-N-oxide (TMAO) levels atherosclerosis most studies. context, CVD CKD through accumulation TMAO, p-Cresyl-sulfate (pCS), indoxyl-sulfate (IS) serum may be result interplay between choline, TMAO microbiota, resulting activation responses fibrosis. Summary A balanced diet, with no nor any outmost importance regarding prevention disorders function. Large-scale studies use next-generation probiotics, especially Akkermansia muciniphila Faecalibacterium prausnitzii , should further examine their therapeutic potential context.

The worldwide surge in obesity and associated metabolic disorders is emerging as a significant public health issue for societies healthcare systems. Available evidence has shown that alterations the gut microbiota could be implicated pathogenesis of disorders. A healthy microbiome characterized by richness high microbial diversity. Gut affect how host responds to diet, conversely, may modify through changes dietary habits. Diet can impact alter composition, diversity, species over time. An unhealthy fat sugar, lead decreased reduced synthesis metabolites maintain permeability, damage mucus layer, increased bacterial translocation lipopolyssacharide which trigger endotoxemia, chronic subclinical inflammation Currently, diet on composition its involvement pathogenic mechanisms underlying one most promising areas research nutrition. This special gathered original articles topics related patterns, microbiota, well brief reports, reviews perspectives wider field translational clinical research. In particular, aim this Special Issue was present connecting with disorders, explore association, examine patterns influence relationship.

ABSTRACT Background Diabetic nephropathy (DN) is a severe microvascular complication of diabetes with complex pathogenesis. Methods Recent studies were reviewed to explore the role gut microbiota and its metabolites in DN development. Results Dysbiosis bacteria contributes pathological changes such as glomerular sclerosis renal tubule injury. Microbial are involved through immune inflammatory pathways. Conclusions Understanding relationship between microbiota, metabolites, may offer potential implications for diagnosis, prevention, treatment. Translating this knowledge into clinical practice presents challenges opportunities.

Gut-derived uremic toxins (UTs) contribute to cardiovascular disorders like atherosclerosis and cardiomyopathy in patients with chronic kidney disease (CKD), causing increased morbidity mortality. The intermediate steps between higher concentrations of gut-derived UTs organ damage caused by are still insufficiently understood. Glycosaminoglycans (GAGs) as components the extracellular matrix known interact various ligands such growth factors or receptors, thereby influencing (patho)physiological processes. We previously found that UT inorganic phosphate (Pi) induces synthesis sulphation GAGs heparan sulphate chondroitin rat vascular smooth muscle cell (VSMC) line A7r5 human endothelial (EC) EA.Hy926. aim this study was investigate if other organic modulate cells well. treated ex vivo cultures aortic rings well primary VSMCs ECs Pi, indoxylsulphate (IS), p-cresylsulphate (pCS), trimethylamine N-oxide (TMAO), urea, analyzed samples histological staining, qPCR, western blot, HPLC, colorimetric assays. treatment contents sulphated hyaluronic acid. UT-treated contained amounts 4S- 6S-sulphated compared controls. This accompanied altered expressions genes proteins relevant for GAG metabolism. Mechanistically, effects on involve activation PI3K/Akt pathway transcription factor NF-κB. In conclusion, UT-induced remodeling upregulation acid tissue might be a missing link pathophysiological alterations system sense gut-matrix axis.

ABSTRACT Postinjury infection continues to plague trauma and emergency surgery patients fortunate enough survive the initial injury. Rapid response systems, massive transfusion protocols, development of level 1 centers, among others, have improved outcome for millions worldwide. Nonetheless, despite this excellent care, still remain vulnerable postinjury infections that can result in organ failure, prolonged critical illness, even death. While risk factors been identified (degree injury, blood loss, time definitive immunocompromise, etc.), they probabilistic, not deterministic, do explain variability at individual case level. Here, we assert analysis social determinants health, as reflected patient's microbiome composition (i.e., community structure, membership) function (metabolomic output), may offer a “window” with which define following traumatic Given emerging knowledge field, more comprehensive evaluation biomarkers within microbiome, from stool-based microbial metabolites those plasma present exhaled breath, when coupled clinical metadata machine learning, could lead deterministic assessment an individual's poor are modifiable.
