The release and absorption profile of an oral medication is influenced by the physicochemical properties drug its formulation, as well anatomy physiology gastrointestinal (GI) tract. During development bioavailability a new typically assessed in early clinical studies healthy adult population. However, many disease conditions are associated with alteration and/or GI same holds true for some subpopulations, such paediatric or elderly patients, populations different ethnicity. variation tract compared to adults can directly affect kinetics absorption, thus, safety efficacy medication. This review provides overview special discusses how affected these conditions. Particular focus directed towards non-disease dependent (age, sex, ethnicity, genetic factors, obesity, pregnancy), diseases (ulcerative colitis Crohn's disease, celiac cancer tract, Roux-en-Y gastric bypass, lactose intolerance, Helicobacter pylori infection, infectious tract), systemic that change (cystic fibrosis, diabetes, Parkinson's HIV enteropathy, critical illness). current knowledge about their impact on still limited. Further research required improve confidence pharmacokinetic predictions dosing recommendations targeted patient population, thus ensure safe effective therapies.

Proteins and peptides (PPs) have gradually become more attractive therapeutic molecules than small molecular drugs due to their high selectivity efficacy, but fewer side effects. Owing the poor stability limited permeability through gastrointestinal (GI) tract epithelia, PPs are usually administered by parenteral route. Given big demand for oral administration in clinical use, a variety of researches focused on developing new technologies overcome GI barriers PPs, such as enteric coating, enzyme inhibitors, permeation enhancers, nanoparticles, well intestinal microdevices. Some been developed under trials even market. This review summarizes history, physiological overcoming approaches, current preclinical technologies, future prospects delivery PPs.

This systematic review aimed to describe medication non-adherence among people living with multimorbidity according the current literature, and synthesise predictors of in this population.A was conducted Preferred Reporting Items for Systematic reviews Meta-Analyses. PubMed, EMBASE, CINAHL PsycINFO were searched relevant articles published English language between January 2009 April 2019. Quantitative studies reporting and/or two or more chronic conditions included review. A meta-analysis a subgroup that used an inclusive definition recruit participants, rather than seeking specific conditions. Remaining prevalence narratively synthesised. The database search produced 10 998 records further 75 identified through other sources. Following full-text screening, 178 The range reported differed by measurement method, at 76.5% self-report, 69.4% pharmacy data, 44.1% electronic monitoring. eight (n=8949) participants. pooled 42.6% (95% CI: 34.0 - 51.3%, k=8, I2=97%, p<0.01). overall 7.0%-83.5%. Transdermal administration remains an active research and development area as alternative route for long-acting drug delivery. It avoids major drawbacks of conventional oral (gastrointestinal side effects, low bioavailability, multiple dosing) or parenteral routes (invasiveness, pain, psychological stress bio-hazardous waste generated from needles), thereby increasing patient appeal compliance. This review focuses on the current state transdermal delivery, including adhesive patches, microneedles, molecularly imprinted polymeric systems. Each subsection describes approach key considerations in formulation development, design, process parameters with schematics. An overview commercially available (adhesive) patches delivery (longer than 24 h), well advanced formulations such core-shell, nanoformulations-incorporated stimuli-responsive 3D-printed polymers is also provided. Finally, we elaborated translational aspects, challenges patch future directions clinical advancement new products
