A significant challenge in cancer treatment is the rising problem of drug resistance that reduces effectiveness therapeutic strategies. Current knowledge shows multiple mechanisms play a role resistance. Another mechanism has gained attention alteration sphingolipid trafficking and dysregulation its metabolism, which was reported to cause cancer-associated Sphingolipids are lipids containing sphingosine have roles, ranging from lipid raft formation, apoptosis, cell signaling immune trafficking. Recent studies show developing cells, altered or dysregulated sphingolipids associated with efflux promote survival cells by bypassing apoptosis. Upregulated levels glucosylceramide synthase (GCS), an enzyme functions lead upregulated ABCB1 gene induces cells. These bypass make drugs induce apoptosis tumor ineffective. By highlighting current findings, this review aims provide caused synthase, kinase, acid ceramidase enzymes as possible targets enhance currently used chemotherapeutic agents.

Sphingolipids, particularly sphingosine-1-phosphate (S1P), are bioactive lipids involved in regulating cellular processes such as proliferation, apoptosis, inflammation, and tumor progression. Alkaline ceramidase 2 (ACER2) plays a critical role sphingolipid metabolism by catalyzing the hydrolysis of ceramide to sphingosine, which is subsequently converted S1P. Dysregulation ACER2 has been implicated various gastrointestinal cancers, including colorectal cancer, gastric hepatocellular carcinoma. ACER2-mediated signaling, through SphK/S1P pathway, influences cancer development modulating immune responses, balance between cell survival death. This review examines physiological functions ACER2, its metabolism, contribution pathogenesis cancers. Understanding mechanisms regulates progression modulation may open new avenues for targeted therapies malignancies.
