Sufficient ocular bioavailability is often considered a challenge by the researchers, due to complex structure of eye and its protective physiological mechanisms. In addition, low viscosity drops resulting short residence time further contribute observed drug concentration at target site. Therefore, various delivery platforms are being developed enhance bioavailability, provide controlled sustained release, reduce number applications, maximize therapy outcomes. Solid lipid nanoparticles (SLNs) nanostructured carriers (NLCs) exhibit all these benefits, in addition biocompatible, biodegradable, susceptible sterilization scale-up. Furthermore, their successive surface modification contributes prolonged (by adding cationic compounds), enhanced penetration, improved performance. The review highlights salient characteristics SLNs NLCs concerning delivery, updates research progress this area.

Disease misdiagnosis is more likely if standardised diagnostic criteria are not used. This study systematically examined the effect on diagnosing dry eye disease (DED), when tests for evaluating tear film homeostasis were included/excluded from a multi-test protocol. For 1427 participants across five sites, data full suite of defined in Tear Film and Ocular Surface Society Dry Eye Workshop II (TFOS DEWS II) Diagnostic Methodology report algorithm evaluated; sensitivity was calculated individual signs removed, different combinations required. Evaluating just one three TFOS homeostatic resulted between 12.3% 36.2% patients who met DED being assigned this diagnosis. While comprehensive ocular surface staining evaluation, including corneal, conjunctival lid margin staining, combination with symptoms had highest (87.7%) markers, dropped to 44.6% only corneal evaluated. Omitting either non-invasive breakup time or osmolarity each by <5%. The prevalence substantially reduced diagnosis required two be present (by 43.7% 61.2%) 65.9% all indicating loss analysis did change significantly differing severities symptoms. plus assessing sign can considered robust appropriate approach

Abstract The purpose of this study is to examine and interpret machine learning models that predict dry eye (DE)-related clinical signs, subjective symptoms, clinician diagnoses by heavily weighting lifestyle factors in the predictions. Machine were trained take assessments ocular surface, eyelids, tear film, combined with symptom scores from validated questionnaire instruments for DE surface diseases, perform a classification into DE-related outcome categories. Outcomes are presented which data-driven algorithm identified subject characteristics, lifestyle, behaviors, or environmental exposures as weighted predictors. Models assessed 5-fold cross-validation accuracy class-wise statistics Age was factor predictions eyelid notching, Line Marx anterior displacement, fluorescein breakup time (FTBUT), well visual analog scale ratings diagnosis blepharitis. Comfortable contact lens wearing ratings. Time spent near work, alcohol consumption, exercise, outdoors predictors several signs symptoms. Exposure airplane cabin environments driving car symptoms but not signs. Prediction accuracies ranged 60.7 86.5%, 73.7 80.1%, 66.9 98.7%. results emphasize importance subject, characteristics etiology disease. Abstract: In the Middle East and Northern Africa (MENA), dry eye disease (DED) is often misdiagnosed or overlooked. This review summarizes a series of conversations with ophthalmologists in region around variety climatic, lifestyle, iatrogenic factors that contribute to specific features DED MENA region. These considerations are further classified by patient lifestyle surgical choices. All statements based on discussions formal voting achieve consensus over three meetings. Overall, deeper understanding characteristics can better guide local eyecare practitioners appropriate management follow-up care.
