Although anxiety can be an adaptive response to unpredictable threats, pathological disorders occur when symptoms adversely affect daily life. Whether or not and share mechanisms remains unknown, but if they do, induced (adaptive) could used as intermediate translational model of improve drug development pipelines. The authors therefore compared meta-analyses functional neuroimaging studies anxiety.A systematic search the PubMed database was conducted in June 2019 for whole-brain MRI articles. Eligible articles contrasted either anxious patients control subjects unpredictable-threat condition a safe healthy participants. Five were included: posttraumatic stress disorder, social generalized panic specific phobia. A total 3,433 records identified, 181 met selection criteria, largest subset task type emotional (N=138). Seed-based d-mapping software all analyses.Induced (N=693 participants) (N=2,554 2,348 subjects) both showed increased activation left right insula (coordinates, 44, 14, -14 -38, 20, -8; k=2,102 k=1,305, respectively) cingulate cortex/medial prefrontal cortex (-12, -8, 68; k=2,217). When analyses split by phobia appeared most, disorder least, similar anxiety. This meta-analysis indicates consistent pattern across anxiety, supporting proposition that some neurobiological overlap former may latter. Induced might nevertheless better than others.

The brain anxiety network is composed of a number interconnected cortical regions that detect threats and execute appropriate defensive responses via projections to the shell nucleus accumbens (NAcSh), dorsolateral region bed stria terminalis (BSTDL) lateral central amygdala (CeL). paraventricular thalamus (PVT) anatomically positioned integrate threat- arousal-related signals from cortex hypothalamus then relay these neural circuits in NAcSh, BSTDL, CeL mediate responses. This review describes anatomical connections PVT support view may be critical node network. Experimental findings are reviewed showing arousal peptides orexins (hypocretins) act at promote avoidance potential especially following exposure rats single episode footshocks. Children with an extremely inhibited, anxious temperament (AT) are at increased risk for anxiety disorders and depression. Using a rhesus monkey model of early-life AT, we previously demonstrated that metabolism in the central extended amygdala (EAc), including nucleus (Ce) bed stria terminalis (BST), is associated trait-like variation AT. Here, use fMRI to examine relationships between Ce–BST functional connectivity AT large multigenerational family pedigree monkeys (n = 170 females 208 males). Results demonstrate heritable, accounts significant but modest portion variance coheritable Interestingly, AT-related BST were not correlated accounted non-overlapping Exploratory analyses suggest hypothalamus periaqueductal gray. Together, these results importance coordinated function within EAc determining individual differences brain regions its behavioral neuroendocrine components. SIGNIFICANCE STATEMENT Anxiety directly impact lives nearly one five people, accounting substantial worldwide suffering disability. nonhuman primate understand neurobiology underlying develop disorders. Leveraging same kinds neuroimaging measures routinely used human studies, activation with, coinherited Understanding how work together produce extreme provides neural target interventions promise preventing lifelong disability at-risk children.

The bed nucleus of the stria terminalis (BNST) has been implicated in conditioned fear and anxiety, but specific factors that engage BNST defensive behaviors are unclear. Here we examined whether mediates freezing to stimuli (CSs) poorly predict onset aversive unconditioned (USs) rats. Reversible inactivation selectively reduced CSs signaled US (e.g., a backward CS followed US), did not eliminate forward even when they predicted USs variable intensity. Additionally, (but forward) increased Fos ventral BNST-projecting neurons infralimbic region medial prefrontal cortex (mPFC), hippocampus or amygdala. These data reveal circuits regulate unpredictable threats, which may be critical etiology expression anxiety.

Oxytocin increases the salience of both positive and negative social contexts it is thought that these diverse actions on behavior are mediated in part through circuit-specific action. This hypothesis based primarily manipulations oxytocin receptor function, leaving open question whether different populations neurons mediate effects behavior. Here we inhibited synthesis a stress-sensitive population specifically within medioventral bed nucleus stria terminalis (BNSTmv). knockdown prevented stress-induced vigilance decreases approach. Viral tracing BNSTmv revealed fibers regions controlling defensive behaviors, including lateral hypothalamus, anterior anteromedial BNST (BNSTam). infusion into BNSTam stress naïve mice increased reduced These results show extrahypothalamic plays key role anxiety behaviors.

When extreme, anxiety—a state of distress and arousal prototypically evoked by uncertain danger—can be debilitating. Uncertain anticipation is a shared feature situations that elicit signs symptoms anxiety across psychiatric disorders, species, assays. Despite the profound significance for human health wellbeing, neurobiology uncertain-threat remains unsettled. Leveraging paradigm adapted from animal research optimized fMRI signal decomposition, we examined neural circuits engaged during temporally certain threat in 99 men women. Results revealed systems recruited are anatomically colocalized frontocortical regions, extended amygdala, periaqueductal gray. Comparison conditions demonstrated this circuitry can fractionated, with regions showing relatively stronger engagement threat, amygdala reverse pattern. Although there widespread agreement bed nucleus stria terminalis dorsal amygdala—the two major subdivisions amygdala—play critical role orchestrating adaptive responses to potential danger, their precise contributions have remained contentious. Follow-up analyses these show statistically indistinguishable anticipation. These observations provide framework conceptualizing fear, understanding functional neuroanatomy humans, accelerating development more effective intervention strategies pathological anxiety. SIGNIFICANCE STATEMENT Anxiety—an emotion associated harm—has public health, yet underlying unclear. novel neuroimaging large sample, identify core circuit responsive both anticipation, fractionated into bias one kind or other. The occupies center stage neuropsychiatric models anxiety, but its architecture has Here demonstrate threat. Collectively, indicate need revise how think about fear.
