MicroRNA‑regulated gene expression plays an important role in autoimmune diseases, such as multiple sclerosis (MS). This study investigated the patterns of microRNAs (miRNAs) MS brain tissues using animal experimental encephalomyelitis (EAE) model treated with Hypericum perforatum (HP) oil. C57BL/6 J mice were divided into two groups: and control. The group was subdivided sham (MS) MS+HP. Multiple sclerosis (MS) is a degenerative condition characterized by axonal damage and demyelination induced autoreactive immune cells that occur in the Central Nervous System (CNS). Multiple sclerosis is a chronic immune-mediated disorder of the central nervous system with high heterogeneity among patients. In clinical setting, one main challenges proper and early diagnosis for prediction disease activity. Abstract Multiple sclerosis (MS) and Neuromyelitis Optica (NMO) are immune-related CNS inflammatory diseases that often present with overlapping clinical symptoms, leading to frequent misdiagnosis, particularly in aquaporin-4 seronegative NMO patients. Identifying the underlying mechanisms of these is critical for discovering biomarkers enable timely diagnosis effective treatment. This study included 252 participants, divided into four groups. Group I (Relapsing–Remitting MS: RRMS group), II (Secondary Progressive SPMS III (NMO IV (Healthy controls). Blood samples were collected from all participants measure expression levels Neuregulin-1 (NRG1), lncRNA Ftx, miR-382-5p . The results showed NRG1 significantly lower RRMS, SPMS, groups compared healthy controls, most pronounced reduction observed NMO, suggesting may serve as a potential biomarker differentiating MS, especially cases where traditional diagnostic criteria inconclusive. Ftx , sponge exhibited an opposite trend was downregulated its promising biomarker. Our findings highlight NRG1, diagnostic, screening, prognostic biomarkers, well tools differential MS. Graphical
