Endovascular treatment is a highly effective therapy for acute ischemic stroke due to large vessel occlusion. However, in clinical practice, nearly half of the patients do not have favorable outcomes despite successful recanalization occluded artery. This unfavorable outcome can be defined as having clinically ineffective reperfusion. The objective review describe reperfusion after endovascular and its underlying risk factors mechanisms, including initial tissue damage, cerebral edema, no-reflow phenomenon, injury, procedural features, variations postprocedural management. Abstract Central nervous system (CNS) accumulation of fibrillary deposits made Amyloid β (A ), hyperphosphorylated Tau or α -synuclein ( -syn), present either alone in the form mixed pathology, characterizes most common neurodegenerative diseases (NDDs) as well aging brain. Compelling evidence supports that acute neurological disorders, such traumatic brain injury (TBI) and stroke, are also accompanied by increased deposition toxic A , -syn species. While contribution these pathological proteins to neurodegeneration has been experimentally ascertained, cellular molecular mechanisms driving -syn-related damage remain be fully clarified. In last few years, studies have shown may contribute inducing and/or promoting blood–brain barrier (BBB) disruption. These can affect BBB integrity directly affecting key components pericytes endothelial cells (ECs) indirectly, macrophages activation dysfunction. Cerebral small vessel disease (cSVD) encompasses a heterogeneous group of age-related pathologies that affect multiple regions. Disease manifestations range from lesions incidentally detected on neuroimaging (white matter hyperintensities, deep infarcts, microbleeds, or enlarged perivascular spaces) to severe disability and cognitive impairment. cSVD accounts for approximately 25% ischemic strokes the vast majority spontaneous intracerebral hemorrhage is also most important vascular contributor dementia. Despite its high prevalence potentially long therapeutic window, there are still no mechanism-based treatments. Here, we provide an overview recent advances in this field. We summarize data highlighting remarkable continuum between monogenic multifactorial cSVDs involving NOTCH3, HTRA1, COL4A1/A2 genes. Taking vessel-centric view, discuss possible cause-and-effect relationships risk factors, structural functional changes, manifestations, underscoring some major knowledge gaps. Although endothelial dysfunction rightly considered central feature cSVD, contributions smooth muscle cells, pericytes, other cells warrant continued investigation.
