Importance

The efficacy of factor XIa inhibition for thromboprophylaxis is unknown. Osocimab a long-acting, fully human monoclonal antibody that inhibits XIa.

Objective

To compare different doses osocimab with enoxaparin and apixaban in patients who have undergone knee arthroplasty.

Design, Setting, Participants

Randomized, open-label, adjudicator-blinded, phase 2 noninferiority trial observer blinding doses, conducted at 54 hospitals 13 countries. Adult undergoing unilateral arthroplasty were randomized from October 2017 through August 2018 followed up until January 2019.

Interventions

Antibody-dependent enhancement (ADE) has been associated with severe disease outcomes in several viral infections, including respiratory infections. In vitro and vivo studies showed that antibody-response to SARS-CoV MERS-CoV could exacerbate infection via ADE. Recently SARS CoV-2, the structural analysis shows a risk of severity This phenomenon is partially attributed non-neutralizing antibodies or at sub-neutralizing levels. These result antigen-antibody complexes' deposition propagation chronic inflammatory process destroys affected tissues. Further, complexes may enhance internalization virus into cells through Fc gamma receptor (FcγR) lead further replication. Thus, ADE occur two mechanisms; 1. Antibody mediated replication 2. Enhanced immune activation. Antibody-mediated effector functions are mainly driven by complement activation, first cascade 1q (C1q) which binds virus-antibody complex.
