Collagens are fundamental constituents of the extracellular matrix and most abundant proteins in mammals. belong to family fibrous or fiber-forming that self-assemble into fibrils define their mechanical properties biological functions. Up now, 28 members collagen superfamily have been recognized. Collagen biosynthesis occurs endoplasmic reticulum, where specific post-translational modification—glycosylation—is also carried out. The glycosylation collagens is very adds β-d-galactopyranose β-d-Glcp-(1→2)-d-Galp disaccharide through β-O-linkage hydroxylysine. Several glycosyltransferases, namely COLGALT1, COLGALT2, LH3, PGGHG glucosidase, were associated with collagens, recently, crystal structure LH3 has solved. At present, safe and effective treatment drugs are urgently needed for diabetic kidney disease (DKD). Circulating protein biomarkers with causal genetic evidence represent promising drug targets, which provides an opportunity to identify new therapeutic targets. Abstract Obesity strongly increases the risk of cardiometabolic diseases, yet underlying mediators this relationship are not fully understood. Given that obesity influences circulating protein levels, we investigated proteins mediating effects on coronary artery disease, stroke and type 2 diabetes. By integrating two-step proteome-wide Mendelian randomization, colocalization, epigenomics single-cell RNA sequencing, identified five prioritized collagen VI α3 (COL6A3). COL6A3 levels were increased by body mass index disease risk. Notably, carboxyl terminus product COL6A3, endotrophin, drove effect. was highly expressed in disease-relevant cell types tissues. Finally, found fat reduction could reduce plasma COL6A3-derived indicating a tractable way to modify endotrophin levels.
