This study aims to analyze global trends in smoking-attributable peptic ulcer disease (PUD) disability-adjusted life years (DALYs) from 1990 2021 and project future 2046. Data were obtained the Global Burden of Disease Study 2021. We calculated age-standardized DALYs rates (ASDR) estimated annual percentage changes (EAPC) for PUD DALYs. Bayesian Age-Period-Cohort models used trends. From 2021, decreased significantly, with rate declining 35.4 9.4 per 100,000 (EAPC: -4.45%). High-income regions showed faster declines, while some low- middle-income countries experienced slower progress or even increases. Gender disparities observed, males consistently showing higher ASDR. Projections suggest a continued decline 2046, persistent regional disparities. By ASDR is expected decrease approximately 3.2 100,000, persisting certain such as Kiribati (44.6 100,000) Cambodia (45.1 100,000). Helicobacter pylori (H. pylori) remains the most-researched etiological factor for gastric inflammation and malignancies. Its evolution towards complications is dependent upon host immune response. Toll-like receptors (TLRs) recognize surface molecular patterns of bacterium, especially lipopolysaccharide (LPS), act pathways, which will finally lead to activation nuclear factor-kappa B (NF-kB), a transcription that stimulates release inflammatory cytokines. MicroRNAs (MiRNAs) finely modulate TLR signaling, but their expression also modulated by NF-kB-dependent pathways. This review aims focus several most researched miRNAs on this subject, with known implications in responses caused H. pylori, including let-7 family, miRNA-155, miRNA-146, miRNA-125, miRNA-21, miRNA-221. TLR–LPS interactions afferent pathways are regulated these miRNAs, can be considered as bridge, connects pre-neoplastic malignant lesions.
