Androgenetic alopecia (AGA) is the most common form of hair loss consisting a characteristic receding frontal hairline in men and diffuse thinning women, with retention, can impact an individual's quality life. The condition primarily mediated by 5-alpha-reductase dihydrotestosterone (DHT) which causes follicles to undergo miniaturization shortening successive anagen cycles. Androgenetic alopecia (AGA) is the most common nonscarring and characterised by distinct gradual patterned hair loss. AGA mediated genetic predisposition excessive follicular sensitivity to androgens, mainly in males, leading progressive conversion of scalp terminal into vellus hair. Although highly prevalent, it not fatal but may have a severe psychosocial impact, especially on females younger males. Significant advances been made understanding AGA's epidemiology pathophysiology, only 2 drugs remain approved FDA - finasteride minoxidil. Prolonged use these drugs, prerequisite for enhanced treatment response. However, this leads poor medication adherence adverse effects from extended eg, "postfinasteride syndrome" which persists beyond stopping drug. Hence, there need research more effective alternative treatments AGA, with fewer side effects. This paper reviewed recent pathophysiology its options. The recently characterized structure type 2, 5-alpha reductase holds significance comprehending present prospective AGA.

Abstract: Androgenetic alopecia (AGA) is the most common cause of hair loss in men and women. Traditionally, topical minoxidil oral finasteride have been standard care yielding mixed results. New treatments such as Low-Level Laser Therapy (LLLT), microneedling, platelet-rich plasma (PRP), others extensively studied literature, purpose this review to provide a comprehensive discussion latest treatment methods their efficacy treating AGA. Novel therapies minoxidil, finasteride, spironolactone, botulinum toxin, stem cell therapy offer interesting alternatives for patients. Treatments for AGA have yet to produce satisfactory outcomes and may cause intolerable side effects. Recent studies reported that adipose tissue-derived stem cell conditioned media (ADSC-CM) could induce hair growth regeneration.To investigate the efficacy of ADSC-CM combined with minoxidil regeneration therapy in male AGA.This study lasted 6 weeks. Subjects were divided into two groups: concentrated non-concentrated ADSC-CM. Scalp was vertically half before intradermal injection administered from frontal region scalp toward vertex a 30G needle, spaced about 1 cm apart. Treatment received 2 ml ADSC-CM; other given NaCl 0.9% as placebo. Patients applied 5% twice daily post-injection. Improvements assessed using photographs trichoscan every weeks.Hair count, density, mean thickness increased significantly on both sides after weeks, while vellus rate decreased proportionally increase terminal rate. No statistically significant differences between treatment groups found. Minimum effects reported, subjects satisfied results.Combination be potential agent regrowth. Introduction: Androgenetic alopecia (AGA) has negative impacts on both men and women in terms of appearance mental stress. Spironolactone is a synthetic aldosterone receptor antagonist known to stimulate hair growth been widely used by dermatologists treat AGA. Objective: To conduct systematic review evaluating the efficacy safety topical oral spironolactone AGA treatment. Methods: We searched PubMed, Embase, Cochrane Library, Web Science until October 23rd, 2022, for human studies treatment AGA, regardless doses routes. Results: retrieved 784 papers ultimately 7 articles matched our inclusion criteria comprised 618 patients (65 men, 553 women), 414 them received Oral ranged from 25mg 200mg daily, with vast majority between 80mg 110 mg. Dosage forms use include gels 1% solutions 5% twice daily. Both have shown recovery, but significantly fewer side effects suitable any gender. It showed better combination other therapies such as or minoxidil compared monotherapy. Conclusion: an effective safe androgenic which can enhance when combined conventional treatments minoxidil. Topical safer than administration male female patients, expected become common drug those who do not good response Furthermore, more high-quality clinical randomized controlled should be performed. Androgenic alopecia (AGA) is the most common type of hair loss, where local high concentrations dihydrotestosterone (DHT) in scalp cause progressive shrinkage follicles, eventually contributing to loss. Due limitations existing methods treat AGA, use multi-origin mesenchymal stromal cell-derived exosomes has been proposed. However, functions and mechanisms action secreted by adipose cells (ADSCs-Exos) AGA are still unclear. Using Cell Counting Kit-8 (CCK8) analysis, immunofluorescence staining, scratch assays, Western blotting, it was found that ADSC-Exos contributed proliferation, migration, differentiation dermal papilla (DPCs) up-regulated expression cyclin, β-catenin, versican, BMP2. also mitigated inhibitory effects DHT on DPCs down-regulated transforming growth factor-beta1 (TGF-β1) its downstream genes. Moreover, high-throughput miRNA sequencing bioinformatics analysis identified 225 genes were co-expressed ADSC-Exos; these, miR-122-5p highly enriched luciferase assays target SMAD3. carrying antagonized inhibition β-catenin versican vivo vitro, restored bulb size thickness, promoted normal follicles. So, enhanced regeneration follicles through TGF-β/SMAD3 axis. These results suggest a novel treatment option for AGA.
