Importance

Chronic back pain (CBP) is a leading cause of disability, and treatment often ineffective. Approximately 85% cases are primary CBP, for which peripheral etiology cannot be identified, maintenance factors include fear, avoidance, beliefs that indicates injury.

Objective

To test whether psychological (pain reprocessing therapy [PRT]) aiming to shift patients’ about the causes threat value provides substantial durable relief from CBP investigate mechanisms.

Design, Setting, Participants

This randomized clinical trial with longitudinal functional magnetic resonance imaging (fMRI) 1-year follow-up assessment was conducted in university research setting November 2017 August 2018, completed by 2019. Clinical fMRI data were analyzed January 2019 2020. The study compared PRT an open-label placebo usual care community sample.

Interventions

Participants participated 1 telehealth session physician 8 sessions over 4 weeks. Treatment aimed help patients reconceptualize their as due nondangerous brain activity rather than tissue injury, using combination cognitive, somatic, exposure-based techniques. received subcutaneous saline injection back; participants continued routine, ongoing care.

Main Outcomes Measures

Results

At baseline, 151 adults (54% female; [SD] age, 41.1 [15.6] years) reported (SD) low moderate severity (mean intensity, 4.10 [1.26] 10; 23.34 [10.12] 100) duration 10.0 (8.9) years. Large group differences observed 1.18 (1.24) group, 2.84 (1.64) 3.13 (1.45) group. Hedgesgwas −1.14 vs −1.74 (P < .001). Of total participants, 33 50 (66%) pain-free or nearly posttreatment (reporting 0 10), 10 51 (20%) 5 (10%) effects maintained follow-up, 1.51 (1.59) 2.79 (1.78) 3.00 (1.77) −0.70 = .001) −1.05 follow-up.
