Recently, the hallmarks of aging were updated to include dysbiosis, disabled macroautophagy, and chronic inflammation. In particular, low-grade inflammation during aging, without overt infection, is defined as "inflammaging," which associated with increased morbidity mortality in population. Emerging evidence suggests a bidirectional cyclical relationship between development age-related conditions, such cardiovascular diseases, neurodegeneration, cancer, frailty. How crosstalk other underlies biological mechanisms disease thus particular interest current geroscience research.

Abstract Some people remain healthier throughout life than others but the underlying reasons are poorly understood. Here we hypothesize this advantage is attributable in part to optimal immune resilience (IR), defined as capacity preserve and/or rapidly restore functions that promote disease resistance (immunocompetence) and control inflammation infectious diseases well other causes of inflammatory stress. Epidemiological evidence indicates that preeclampsia (PE) is associated with comorbidities such as periodontitis (PD). However, the underlying mechanism remains unclear. To enhance our understanding of their co-pathogenesis, this research investigated shared biomarkers and pathological mechanisms. We systematically retrieved transcriptomic datasets from Gene Expression Omnibus database. These encompass a comparative analysis periodontium without PD placenta PE. Differentially Expressed Genes Analysis Weighted Go-expression Network (WGCNA) were used to identify key crosstalk genes in patients The functional characterisation these was performed using enrichment analysis. Importance

Preventive strategies for frailty are needed. Whether supplements with anti-inflammatory properties, such as vitamin D3or marine omega-3 fatty acids, useful prevention is unknown.

Objective

To test the effects of D3and on change in older individuals.

Design, Setting, and Participants

This study was conducted 2021, a prespecified ancillary to Vitamin D Omega-3 (VITAL) trial, 2 × factorial randomized clinical trial. A total 25 871 individuals (men aged ≥50 years women ≥55 years), without cancer or cardiovascular disease data frailty, were recruited across all 50 US states from November 2011 March 2014 followed up through December 31, 2017. Data analysis 1, 2019, 30, 2022.

Interventions

D3, 2000 IU/d, 1 g/d.

Main Outcomes Measures

Frailty measured using validated 36-item index that includes measures function, cognition, mood, comorbidities annual questionnaires. Change score baseline year 5, according randomization, an intention-to-treat protocol, assessed repeated measures. Cox proportional hazards regression models incident frailty. In subgroup analysis, alternative definition, physical phenotype, used sensitivity analysis.

Results

Of VITAL trial participants randomized, 057 had sufficient calculate index. Baseline mean (SD) age 67.2 (7.0) years, 12 698 (50.7.%) women. Mean 0.109 (0.090) (range, 0.00-0.685), 3174 (12.7%) frail. During median 5-year follow-up, scores increased 0.121 (0.099) 0.00-0.792). Neither D3nor acid supplementation affected over time (mean difference at 5: −0.0002;P = .85; acid, −0.0001;P .90) rate (interaction time: D3;P .98; acid;P .13) Incident remained similar D3,P .90; .32). Results unchanged phenotype.

Conclusions Relevance

this treatment supplementation, compared placebo, did not affect incidence time. These results do support routine use either generally healthy community-dwelling adults selected D3deficiency.
