Importance Prospective and retrospective measures of childhood maltreatment identify largely different groups individuals. Importance Adverse life experiences have been proposed to contribute diverse mental health problems through an association with corticolimbic functioning. Despite compelling evidence from animal models, findings studies in humans mixed; activation likelihood estimation (ALE) meta-analyses failed identify a consistent of adverse events brain function. Estimating the current association between childhood adversity and risk of psychosis is crucial for prevention intervention. We provided an updated synthesis evidence from past four decades, expanded available data by investigating a broad array subtypes, explored sex differences age onset as relevant factors. searched PubMed, EMBASE, PsycINFO, Web Science, WANFANG, CNKI, case-control, cross-sectional cohort studies on psychotic symptoms/illness. Multi-level meta-analysis, prediction intervals calculation, sensitivity analyses were conducted. The main analysis included 183 study samples (N=349,265), with 119 case-control (15,186 cases; 14,879 controls), 51 (N=299,659), 13 (N=19,541). Significant associations observed across all designs, yielding overall odds ratio 2.80 (95% CI=2.18, 3.60). Secondary revealed that exposure to each subtype increased psychosis, highest (3.54 [95% CI=3.04, 4.13]) emotional abuse, lowest (1.58 CI=1.48, 1.68]) parental antipathy. No statistically significant observed, although sexual abuse was higher women. Onset earlier in adversity-exposed individuals (mean difference=-0.79 years, 95% CI=-1.47 -0.12).
