Disparities in mental health have persisted or worsened despite our awareness of their existence, increased understanding causes, and efforts at reduction mitigation. Although much is known, there still to be done research meaningfully impact disparities. In November 2020, the National Institute Mental Health (NIMH) Minority (NIMHD) co-sponsored a virtual workshop explore complexities disparities, which revealed several gaps opportunities for field pursue advance disparities research. This article, introduction Special Issue on Disparities, provides frame four articles that stem from are inspired by NIMH/NIMHD workshop, all illustrate innovative complex mechanisms how this knowledge can translated into effective intervention development advances equity.

This meta-analysis synthesizes experimental studies on the immediate effects of discrimination mental health, exploring different paradigms and types diverse facets health. We analyzed data from a systematic literature search (73 studies; 12,097 participants; 245 effect sizes) for randomized controlled trials with manipulation as predictor health an outcome using three-level random-effects model.Experimentally manipulated led to poorer (g = -0.30),also after controlling publication year, region, education level, methodological quality.Moderator analyses revealed stronger pervasive -0.55)compared single-event manipulations -0.25)and trend toward weaker samples nonmarginalized -0.16)compared marginalized identities -0.34).Gender age did not moderate effect.Discrimination had largest externalizing -0.66)and distress-related outcomes -0.41);heterosexism -0.66),racism -0.32),and sexism -0.30)had health.Convenience sampling compromised generalizability subgroups general population, downgrading quality all included studies. When interpreting findings, selective (mostly young female adults higher education), often limited ecological validity, ethical restrictions lab-induced need be considered. These constraints likely conservative estimates in this meta-analysis.Future research should investigate more samples, further explain heterogeneity explore protective factors health.

Exposure to per- and poly-fluoroalkyl substances (PFAS) remains an important public health issue due widespread detection persistence in environmental media, slow metabolism humans, influences on physiological processes such as neurological signaling. Maternal depression is highly prevalent during pregnancy postpartum potentially sensitive PFAS. The risks associated with PFAS may be further amplified historically marginalized communities, including immigrants.

Cystic fibrosis (CF) has been traditionally viewed as a disease that affects White individuals. However, CF occurs among all races, ethnicities, and geographic ancestries. The disorder results from mutations in the transmembrane conductance regulator (CFTR). Varying incidence of is reported Black, Indigenous, People Color (BIPOC), who typically exhibit worse clinical outcomes. These populations are more likely to carry rare CFTR variants omitted newborn screening panels, leading disparities care such delayed diagnosis treatment. In this study, we present case-in-point describing an individual Gambian descent identified with CF. Patient genotype includes premature termination codon (PTC) (c.2353C>T) previously undescribed single nucleotide deletion (c.1970delG), arguing against effectiveness currently available modulator-based interventions. Strategies for overcoming these two will include combinations PTC suppressors, nonsense mediated decay inhibitors, and/or alternative approaches (e.g. gene therapy). Investigations study establish foundation which therapeutic treatments may be developed. Importantly, c.2353C>T c.1970delG were not detected patient by traditional implicit racial ethnic diagnostic bias tests comprised largely observed people European ancestry. We suggest next-generation sequencing should utilized confirm or exclude diagnosis, order equitably serve BIPOC Additional epidemiologic data, basic science investigations, translational work imperative improving understanding prevalence progression, variant frequency, genotype-phenotype correlation, pharmacologic responsiveness, personalized medicine patients African ancestry other historically understudied lineages.
