BACKGROUND Violence against paramedics has been described as a serious public health problem with the potential for significant physical and psychological harm, but organizational culture within profession encourages to consider violence just “part of job.” Therefore, most incidents are never formally documented. This limits ability researchers policy makers develop strategies that mitigate risk enhance paramedic safety. OBJECTIVE Following development implementation novel, point-of-event reporting process in February 2021, our objectives (1) estimate prevalence generate descriptive profile reported violence; (2) identify potentially high-risk service calls based on characteristics generally known responding at point dispatch; (3) explore underpinning themes, including intolerance gender, race, sexual orientation, contribute violence. METHODS Our work is situated single Ontario, Canada. Using convergent parallel mixed methods approach, we will retrospectively review 2 years quantitative qualitative data gathered from External Incident Report (EVIR) system 1 2021 through 28, 2023. Public safety personnel (PSP) are frequently exposed to potentially psychologically traumatic events (PPTEs) which can impact mental health. To help mitigate the negative effects of PPTEs, PSP commonly rely on peer support. Peer support generally refers a wide variety health resources that offer social or emotional assistance peer, and in some cases professional assistance. Violence against paramedics is increasingly recognized as an important occupational health problem, but pervasive and institutionalized underreporting hinders efforts at risk mitigation. Earlier research has shown that the organizational culture within paramedicine may contribute to researchers have recommended involving in development of violence prevention policies, including reporting systems. Eighteen months after launch a new comprehensive system Peel Region, Ontario, Canada, we surveyed about their willingness report violent encounters. A total 204 (33% eligible) chose participate, whom 67% (N=137) had experienced since process, with 83% (N=114) incidents least some time. In choosing report, participants cited accessibility process desire promote accountability among perpetrators while contributing safer workplace motivating factors. Their decisions file however, could be influenced by perceived ‘volitionality’ severity encounters, particularly context (un)supportive co-workers supervisors. Ultimately, participants’ belief would lead meaningful change service was key driver behavior.

Managing post-traumatic stress reactions in the first few days after exposure to a potentially traumatic event course of one’s work remains challenge for responder organizations such as Emergency Medical Services (EMS). Psychological First Aid (PFA) is an evidence-informed approach reducing initial distress and promoting short- long-term coping strategies among staff aftermath exposure. PFA provided by peer helpers considered promising solution organizations. Unfortunately, responders may encounter stigma barriers mental health care. Therefore, deeper investigation needed regarding adherence over time implemented intervention. The purpose this study qualitatively explore factors that influence intervention recipients helpers. EMS workers (n = 11), working one year, participated semi-structured interviews. Data were analyzed using thematic analysis; intercoder reliability (κ 0.91) was also used. Researchers identified four themes 11 subthemes influencing intervention: (1) individual perceptions attitudes about pfa intervention; (2) perceived impacts on recipients; (3) organizational support (4) congruence with occupational culture. Study findings herein suggest it conceivable act various improve within organization. This could lead enhanced understanding challenges involved sustaining led program responders.

Violence against paramedics has been described as a ‘serious public health problem’ but one that remains ‘vastly underreported’, owing to an organizational culture stigmatizes reporting – hindering efforts at risk mitigation in addition creating gap research. Leveraging novel process developed after extensive stakeholder consultation and embedded within the electronic patient care record, our objective was provide descriptive profile of violence single paramedic service Ontario, Canada. Between February 1, 2021, through January 31, 2023, total 374 Peel Region (48% workforce) generated 941 reports, which 40% documented physical (n=364) or sexual (n=19) assault. The typically perpetrated by patients (78%) primarily took place scene 9-1-1 call (47%); however, violent behavior frequently persisted recurred while transit hospital arrival. Collectively, mental health, alcohol, drug use were listed contributing circumstances 83% reports. In all, 81 physically harmed because On average, data correspond filing report every 18 hours, being assaulted 46 injured 9 days.

Melatonin (MT) is an indoleamine hormone that can counteract ischemia‑induced organ injury through its antioxidant effects. The aim of the present study was to investigate protective effects exogenous MT against hemorrhagic shock (HS)‑induced hepatic ischemic in rats, and role nuclear factor (NF)‑κB signaling pathway this process. A rat model HS‑induced established. serum levels alanine aminotransferase (ALT), aspartate (AST), lactate dehydrogenase (LDH), glutamate (GDH), tumor necrosis (TNF)‑α, interferon (IFN)‑γ, interleukin (IL)‑6 IL‑1β were measured every 6 h, 24‑h survival rate rats analyzed. All surviving sacrificed after 24 h. Pathological changes liver hepatocyte apoptosis observed by hematoxylin eosin staining TUNEL assay, respectively, expression NF‑κB p65 inhibitor α (IκBα) analyzed reverse transcription‑quantitative PCR analysis western blotting. results demonstrated ALT, AST, LDH, GDH, TNF‑α, IFN‑γ, IL‑6 gradually increased HS compared with those subjected a sham procedure, but increase attenuated MT. Furthermore, group significantly higher group. degree pathological injury, rate, decreased In addition, mRNA IκBα protein cytosol nucleus, while phosphorylated Therefore, it may be inferred alleviates via inhibition activation phosphorylation.

There is growing recognition in research and policy of a mental health crisis among Canada’s paramedics but despite this, epidemiological surveillance the problem its infancy. Just weeks before emergence COVID-19 pandemic, we surveyed from single, large, urban paramedic service Ontario, Canada to assess for symptom clusters consistent with Post-Traumatic Stress Disorder (PTSD), major depressive disorder, generalized anxiety disorder identify potential risk factors each. In total, received 589 completed surveys (a 97% completion rate) found that 11% screened positive PTSD, 15% depression, anxiety, 1 4 active-duty screening any three as recently February 2020. adjusted analyses, screen varied function employment classification, gender, self-reported resilience, previous experience member service’s peer support team. Our findings position disorders at high rates – likely have worsened since onset pandemic. We echo calls researchers policymakers urgent action Canada.
