One fifth of the world population live in East Asia comprising Japan, Korea, and China where ischemic heart disease, a major component atherosclerotic cardiovascular disease (ASCVD), is second most frequent cause death. Each low-density lipoproteins (LDL), remnant lipoproteins, lipoprotein(a), summarized as non-high-density (non-HDL) or apolipoprotein B (apoB) containing causes ASCVD. However, significant proportion evidence on lipoprotein cholesterol with risk ASCVD came from White people mainly living Europe North America not Asian descent. With unique biological, geohistorical, social background this region, Asians have distinctive characteristics that might potential impact association Considering movement across national borders World, understanding important for both non-East populations wherever they World.

Purpose of review Remnant cholesterol has become increasingly recognized as a direct contributor to the development atherosclerosis and an additional marker cardiovascular risk. This aims summarize pathophysiological mechanisms, current evidence base from epidemiological investigations genetic studies that support causal link between remnant atherosclerotic disease. Current novel therapeutic approaches target are discussed. Recent findings A recent Mendelian randomization study over 12 000 single-nucleotide polymorphisms associated with high levels cholesterol, demonstrated association adverse events among 958 434 participants. Summary In this light, emerging role independent lipid risk warrants reevaluation management guidelines underscores potential for targets in disease prevention.

Atherosclerotic cardiovascular disease (ASCVD) is the world's leading cause of death. ASCVD has multiple mediators that therapeutic interventions target, such as dyslipidaemia, hypertension, diabetes and heightened systemic inflammatory tone, among others. LDL cholesterol one most well-studied established targeted for primary secondary prevention ASCVD. However, despite strength evidence supporting reduction by management strategies, events can still recur, even in patients whose been very aggressively reduced. Hypertriglyceridaemia elevated levels triglyceride-rich lipoproteins (TRLs) may be key contributors to residual risk. Several observational genetic epidemiological studies have highlighted causal role triglycerides within TRLs and/or their remnant development progression consist intestinally derived chylomicrons hepatically synthesised LDL. Lifestyle modification considered first line intervention managing hypertriglyceridaemia. Multiple novel therapies are development, shown efficacy preclinical clinical phases study hypertriglyceridaemia TRLs. This comprehensive review provides an overview biology, pathogenicity, epidemiology, genetics TRLs, how they impact risk In addition, we provide a summary currently available emerging triglyceride-lowering development.

Atherosclerotic cardiovascular disease (ASCVD) is the leading cause of morbidity and mortality, being twofold to fourfold more common in patients with type 2 diabetes mellitus (T2DM) than individuals without diabetes. However, despite this decade-old knowledge, identification a specific prognostic risk biomarker remains particularly challenging.Taking advantage large sample Caucasian (n = 529) diagnosis T2DM followed for median 16.8 years, present study was aimed at testing hypothesis that fasting serum proprotein convertase subtilisin/kexin 9 (PCSK9) levels could be major adverse events (MACE) all-cause mortality.Median PCSK9 were 259.8 ng/mL, higher women compared men increasing even presence complication (e.g., diabetic kidney disease). positively correlated markers blood glucose homeostasis HbA1c, insulin HOMA-IR) atherogenic lipid profile non-HDL-C, apoB remnant cholesterol). Abstract Background The effect of remnant-cholesterol (remnant-C) on incident end-stage renal disease (ESRD) has not been studied longitudinally. This retrospective cohort study evaluated the association between remnant-C and development ESRD in a nationwide Korean cohort. Methods Participants National Health Insurance Service health examination ( n = 3,856,985) were followed up until onset ESRD. median duration follow-up was 10.3 years. The current first-line treatment for atherosclerotic cardiovascular disease (ASCVD) involves the reduction of a patient’s low-density lipoprotein cholesterol (LDL-C) levels through use lipid-lowering drugs. However, even when other risk factors such as hypertension and diabetes are effectively managed, there remains residual in these patients despite achieving target LDL-C with statins new medications. This was previously believed to be associated lipid components than LDL, triglycerides. recent studies have unveiled crucial role remnant (RC) atherosclerosis, not just metabolized product triglyceride-rich lipoproteins is referred particles, its component known RC. Numerous pieces evidence from epidemiological investigations genetic demonstrate that RC plays significant predicting incidence ASCVD. As novel marker atherosclerosis prediction, appropriately controlled, should prioritized attention intervention among individuals at high Therefore, reducing various drugs may yield long-term benefits. Nevertheless, routine testing clinical practice controversial, necessitating further research on elevated evaluate advantages

Previous findings have indicated that elevated low-density lipoprotein cholesterol (LDL-C) and remnant (RC) are associated with hypertension. We aim to explore whether higher RC levels may be hypertension beyond LDL-C in the general US adult population. This study included 10,842 adults from National Health Nutrition Examination Survey (NHANES) 1999-2018. Weighted multivariable logistic regression models were used estimate odds ratios (ORs) of for RC. also performed analyses examining association between vs. concordant/discordant groups.A total 4,963 (41.54%, weighted) individuals had The weighted median LDL-C: 118mg/dL, RC: 20mg/dL. At lower clinical cut-point, proportion discordantly high dramatically increased. After adjustment, log was prevalence [OR 2.54, 95% confidence interval (CI) 2.17-2.99]. Participants highest tertile more likely (OR 2.18; CI 1.89-2.52) compared those lowest This remained marked after including body mass index (BMI), LDL-C, high-density (HDL-C) or triglycerides. absent adjusting BMI, Compared low LDL-C/low group, discordant LDL-C/high group 2.04; 1.72-2.42), whereas not, regardless HDL-C Similar results observed when discordance among different cut-points, except cut-point 70 mg/dL 13 mg/dL. To better understand association, we an additional analysis, which showed participants apolipoprotein B < (92mg/dL), ≥ (20mg/dL) significantly having 1.73; 1.38-2.17).RC population. went increased triglycerides levels, lipoproteins other than apoB involved.
