Among hypertensive cohorts across different nations, the relationship between triglyceride-glucose index (TyG) and its conjunction with obesity metrics in relation to cardiovascular disease (CVD) incidence mortality remains be elucidated. This study enrolled 9,283, 164,357, 5,334 hypertensives from National Health Nutrition Examination Survey (NHANES), UK Biobank (UKBB), Shanghai Pudong cohort. The related outcomes for CVD were defined by multivariate Cox proportional hazards models, Generalized Additive Models Mendelian randomization analysis. Mediation analysis explored mediating role of inflammatory markers above relationships. Five measures insulin resistance linked death a U-shaped pattern, highest group having risk increases. Higher glucose triglyceride-waist height ratio (TyG-WHTR) was higher all-cause (UKBB: HR 1.21, 95%CI 1.16–1.26, NHANES: 1.17, 1.00–1.36), 1.36, 1.23–1.49, 1.32, 1.00–1.72) risks. In China cohort, triglyceride/high-density lipoprotein-cholesterol (TG/HDL_C) associated risks stroke (HR 1.31, 1.00–1.73 1.67, 1.06–2.63). Inflammation like systemic response (SIRI) C-reactive protein (CRP) partially explained these links, CRP stronger effect. Genetically predicted TyG also (OR 1.26, 1.10–1.45) risk. An elevated indices are significantly correlated an increased three national cohorts. These anticipated serve as valid predictors incident individuals hypertension.

Triglyceride-glucose (TyG) related indices, which serve as simple markers for insulin resistance, have been closely linked to metabolic dysfunction-associated steatotic liver disease (MASLD), cardiovascular (CVD), and mortality. However, the prognostic utility of TyG-related indices in predicting risk CVD mortality among patients with MASLD remains unclear. Data 97,331 patients, a median age 58.0 years free at baseline, were obtained from UK Biobank. The TyG index, along its combination adiposity parameters (i.e. body mass index [BMI], waist circumference [WC], waist-to-height ratio [WHtR]), calculated. Cox proportional hazards models restricted cubic spline (RCS) performed evaluate associations between overall CVD, coronary heart (CHD), stroke, all-cause mortality, Additionally, Harrell's C-index, net reclassification (NRI), integrated discrimination improvement (IDI) used assess predictive performance these indices. Over follow-up 13.56 years, we identified 13,256 cases 10,980 CHD, 2,926 8,809 1,796 Compared lowest quartile participants high had significantly increased incident Specifically, hazard ratios occurring fourth versus first quartiles 1.19 (95% confidence interval: 1.13-1.25) TyG, 1.35 (1.28-1.42) TyG-BMI, 1.33 (1.26-1.40) TyG-WC, 1.39 (1.32-1.46) TyG-WHtR. RCS analyses indicated nonlinear association outcomes (all P values nonlinearity < 0.05), whereas there exhibited linear trends TyG-WHtR > 0.05, except TyG-WC stroke). Furthermore, demonstrated higher NRI, IDI patients. especially TyG-WHtR, significant individuals MASLD. may effective surrogate predictors

Accurate prediction of cardiovascular disease (CVD) mortality is essential for effective treatment decisions and risk management. Current models often lack comprehensive integration key biomarkers, limiting their predictive power. This study aims to develop a model CVD-related using machine learning-based feature selection algorithm assess its performance compared existing models. We analyzed data from cohort 4,882 adults recruited between 1999 2004, followed up 20 years. After applying the Boruta selection, biomarkers including NT-proBNP, cardiac troponins, homocysteine were identified as significant predictors CVD mortality. Predictive built these alongside demographic clinical variables. Model was evaluated concordance index (C-index), sensitivity, specificity, accuracy, with internal validation conducted through bootstrap sampling. Additionally, decision curve analysis (DCA) performed benefit. The combined model, incorporating both variables, demonstrated superior C-index 0.9205 (95% CI: 0.9129–0.9319), outperforming variables alone (C-index: 0.9030 0.8938–0.9147)) or 0.8659 0.8519–0.8826)). Cox regression further mortality, elevated AST/ALT, TyG, BUN, systolic blood pressure, protective factors such higher chloride iron levels. Nomogram construction DCA confirmed that provided substantial net benefit across various time points. lipid profiles, inflammatory markers significantly improves accuracy novel approach offers enhanced prognostication, potential optimization inclusion additional lifestyle data.
