Background: Diabetic macular edema (DME) is the leading cause of vision impairment in diabetic patients, with intravitreal anti-vascular endothelial growth factor (anti-VEGF) injections being first-line therapy. However, one-third patients exhibit persistent DME despite treatment, suggesting additional pathogenic factors. This study aimed to evaluate predictive value complete blood count (CBC)-based inflammation indexes and optical coherence tomography (OCT) parameters determining early anti-VEGF treatment effectiveness DME. Methods: One hundred four naïve DME, treated 0.05 mL aflibercept were retrospectively analyzed. Blood analyzed included neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte (MLR), platelet-to-lymphocyte (PLR), systemic immune-inflammation index (SII). Baseline OCT biomarkers subretinal fluid (SRF), intraretinal cysts (IRC), hyperreflective retinal spots (HRS), disorganization inner layers (DRIL). Treatment response was defined as a minimum 10% reduction central thickness (CMT) at one month post-injection. Results: NLR, MLR, PLR, SII significantly higher non-responders (p < 0.001), but their fair, an area under ROC curve ranging between 0.704 (MLR) 0.788 A multivariate model including SII, initial CMT, presence IRC showed excellent prediction for anatomical (AUC 0.911). At same time, lower DRIL, SRF, absence HRF correlated gain BCVA. Conclusions: CBC-derived indices have prognostic predicting therapy terms functional outcomes. These findings could help identify poor responders guide personalized strategies.

Non-alcoholic fatty liver disease stands as the predominant cause of chronic disease, with its prevalence and morbidity expected to escalate significantly, leading substantial healthcare costs diminished health-related quality life. It comprises a range manifestations that commence basic steatosis, involving accumulation lipids in hepatocytes, distinctive histological feature. If left untreated, it often advances non-alcoholic steatohepatitis, marked by inflammatory and/or fibrotic hepatic changes, eventual development disease-related cirrhosis hepatocellular carcinoma. Because liver's vital role body metabolism, is considered both consequence contributor metabolic abnormalities observed syndrome. As date, there are no authorized pharmacological agents for or steatohepatitis. Semaglutide, glycemic weight loss advantages, could potentially offer benefits individuals disease. This review aims investigate impact semaglutide on

Vascular calcification (VC) commonly occurs in diabetes and is associated with cardiovascular disease incidence mortality. Currently, there no drug treatment for VC. The Danlian-Tongmai formula (DLTM) a traditional Chinese medicine (TCM) prescription used diabetic VC (DVC), but its mechanisms of action remain unclear. This study aims to elucidate the effects DLTM on DVC explore underlying action. Ultra-high-performance liquid chromatography-mass spectrometry (UHPLC-MS) was identify metabolites DLTM. A rat model established using streptozotocin (STZ) combined vitamin D3 (VitD3). were evaluated through alizarin red staining, calcium deposition, changes osteogenic contractile markers. specific molecular mechanism treating comprehensively analyzed by transcriptomics, docking vivo experimental verification. We identified 108 major In vivo, high-dose significantly alleviated rats. Transcriptomic analysis showed that markedly altered transcriptomic profile aortas, which regulating CCN3/NOTCH signaling pathway, promoting vascular smooth muscle contraction, inhibiting inflammatory responses. Molecular dynamics simulation demonstrated strong binding interactions between key molecules within including NOTCH1, DLL1, DLL4, hes1, hey1. experiments confirmed could upregulate CCN3, inhibit activation NOTCH ligands DLL1 downstream transcription factors hes1 hey1, reduce release cytokines IL6, IL1β, TNFα. alleviates axis Our research provides basis clinical transformation
