Direct interventions into deep brain circuits constitute promising treatment modalities for chronic pain. Cingulotomy and stimulation targeting the anterior cingulate cortex have shown notable improvements in unpleasantness of pain, but these require surgeries. In this study, we developed an approach that can modulate affective hub entirely noninvasively, using low-intensity transcranial-focused ultrasound. Twenty patients with pain received two 40-minute active or sham protocols were monitored one week a randomized crossover trial. Sixty percent subjects experienced clinically meaningful reduction on day 1 7 following stimulation, while provided such benefits only to 15% 20% subjects, respectively. On average, reduced by 60.0% immediately intervention 43.0% 33.0% days intervention. The corresponding levels 14.4%, 12.3%, 6.6%. was well tolerated, no adverse events detected. Side effects generally mild resolved within 24 hours. Together, direct, ultrasonic offers rapid, meaningful, durable severity.

Low-intensity transcranial focused ultrasound (tFUS) has recently been shown to noninvasively and non-pharmacologically modulate pain hypersensitivity with high spatial specificity deep brain penetration. However, the lack of knowledge about its effectiveness for management in older subjects vulnerable severe who are also at increased risk cognitive impairment, presents significant challenges. Additionally, current opioid treatments require hospital visits, limiting unwanted serious side effects multiple liabilities, device-based typically administered medical facilities bulky expensive equipment, accessibility thus highlighting need at-home non-pharmacological treatment options. Here, we present a more accessible, noninvasive tFUS strategy senior subjects. This approach involves simultaneously targeting pain-processing circuits using battery-powered, compact, low-cost analog front end (UAFE). We developed evaluated performance UAFE capable generating sufficiently high-amplitude output significantly lower noise levels compared commercial transmitter. Using humanized sickle mouse model chronic hyperalgesia, found that stimulation effectively reduces heat hyperalgesia aged female mice. In addition efficacy, our behavioral-based safety assessment revealed no adverse on motor functions. These results suggest compact target can suppress pain-related behaviors mice without negatively impacting coordination balance. highlights potential further development fully home-based seniors.

ABSTRACT Dopamine is a neurotransmitter that plays significant role in reward and motivation. Dysfunction the mesolimbic dopamine pathway has been linked to variety of psychiatric disorders, including addiction. Low‐intensity focused ultrasound (LIFU) demonstrated effects on brain activity, but how LIFU affects neurotransmission not known. Here, we applied three different intensities (6.5, 13, 26 W/cm 2 I SPPA ) 2‐min prelimbic cortex (PLC) measured nucleus accumbens (NAc) core using fast‐scan cyclic voltammetry. Two minutes sonication at 13 PLC significantly reduced release by ~50% for up h. However, double intensity (26 resulted less inhibition (~30%), half (6.5 did result any dopamine. Anatomical controls applying primary somatosensory change NAc dopamine, affect caudate or shell. Histological evaluations showed no evidence cell damage death. Modeling temperature rise demonstrates maximum 0.5°C with , suggesting modulation due thermal mechanisms. Abstract Transcranial focused ultrasound (tFUS) is an emerging noninvasive neuromodulation technology that has shown great potential in pain modulation. This review systematically elucidates the multilevel biological mechanisms of tFUS neuromodulation, from network-wide effects to cellular and molecular processes, as well broader systemic influences.
