Although tinnitus represents a major global burden, no causal therapy has yet been established. Ongoing controversies about the neuronal pathophysiology of hamper efforts in developing advanced therapies. Hypothesizing that unnoticed co-occurrence hyperacusis and differences duration may possibly differentially influence neural correlate tinnitus, we analyzed 33 patients without (T-group) 20 with (TH-group). Tinnitus is the phantom perception of sound when there no external auditory input. This mostly perceived as a ringing, whistling or buzzing in ear. Noise-induced cochlear synaptopathy (CS) is defined as a permanent loss of synapses in the auditory nerve pathway following noise exposure. Several studies using brainstem response (ABR) have indicated presence CS and increased central gain tinnitus patients with normal hearing thresholds (TNHT), but results were inconsistent. Introduction This study evaluates cochleovestibular implants (CVI) for improving auditory and vestibular function in patients with bilateral vestibulopathy severe-to-profound hearing loss. CVI uniquely combines stimulation, offering a potential solution dual impairments balance. Methods Ten underwent implantation. Auditory was assessed pure-tone audiometry (PTA) speech recognition at baseline, 3, 9 months post-implantation. Vestibular measured using the Dynamic Gait Index (DGI), Sensory Organization Test (SOT), posturography, Dizziness Handicap Inventory (DHI). Placebo-controlled tests verified stimulation specificity. Results outcomes showed PTA improvements from 78 dB HL preoperatively to 34 3 36 months, alongside gains. included significant DGI ( p < 0.05) SOT score increases (33% 68%, 0.05).
