Abstract While geographic search filters exist, few of them are validated and there currently none that focus on Germany. パーキンソン病 (以下, PD) は高齢者に多く発症する神経変性疾患であり, その患者数は超高齢社会に伴い, 爆発的な増加傾向にある。PD 患者の QOL を阻害する重要な因子の 1 つとして, 嚥下障害がある。PD の嚥下障害は高頻度にみられ, 必ずしも重症度と相関しない。嚥下障害に関連する症状として, 体重減少, 誤嚥性肺炎, 流涎などが挙げられる。大脳基底核回路や延髄嚥下中枢以外でも様々な病変部位によって, PD の嚥下障害はすべての摂食嚥下ステージ (先行期, 準備期, 口腔期, 咽頭期, 食道期) で起こりえる。なかでも咽頭期での障害が多いとされている。PD 患者は嚥下障害の自覚に乏しく不顕性誤嚥も多いことから, 嚥下障害の訴えがない 患者にも問診を行い, 嚥下障害が疑われる場合には積極的に嚥下造影検査や嚥下内視鏡検査で嚥下障害を評価する。嚥下障害のパターンから病変部位を推定し治療方針を決定することが重要である。
