One in ten adults Europe have chronic insomnia, which is characterised by frequent and persistent difficulties initiating and/or maintaining sleep daily functioning impairments. Regional differences practices access to healthcare services lead variable clinical care across Europe. Typically, a patient with insomnia (a) will usually present primary physician; (b) not be offered cognitive behavioural therapy for insomnia—the recommended first-line treatment; (c) instead receive hygiene recommendations eventually pharmacotherapy manage their long-term condition; (d) use medications such as GABA receptor agonists longer than the approved duration. Available evidence suggests that patients multiple unmet needs, actions clearer diagnosis of effective management this condition are long overdue. In article, we provide an update on Old new treatments summarised information indications, contraindications, precautions, warnings, side effects. Challenges treating European systems, considering patients’ perspectives preferences presented discussed. Finally, suggestions provided—with providers policy makers mind—for strategies achieve optimal management.

Obstructive sleep apnea (OSA) is a common disorder that causes intermittent hypoxia and fragmentation, leading to attention impairment other cognitive deficits. Magnetic resonance imaging (MRI) powerful modality can reveal the structural functional brain alterations associated with in OSA patients. The objective of this systematic review identify synthesize evidence on MRI biomarkers neuropsychological assessments deficits patients. We searched Scopus PubMed databases for studies used measure related alteration patients reported qualitative quantitative data association between outcomes. Summary The cyclic alternating pattern (CAP) is a variation of sleep electroencephalogram activity within non‐rapid eye movement (non‐REM) and increased CAP the marker instability. present study aimed to examine expression in non‐REM patients with systemic sclerosis (SSc) compare these findings control group. Thirty‐one SSc were compared matched controls by age, gender, body mass index, apnea–hypopnea index (AHI). Sleep measurements obtained standard polysomnography conventional scoring. A analysis was performed verified certified somnologists. parameters groups similar for efficiency, stage percents N1, N2, N3, latency, minimum oxygen saturation ( p = 0.627, 0.693, 0.530, 0.736, 0.772, respectively). presented higher arousal periodic limb index. analyses showed that had numbers, phase A2 A3 duration, lower A1 duration 0.032, 0.016, Overall total time during significantly patients, although efficiency distribution macro‐structure seem be This emphasises microstructure provides information would otherwise lost if only macrostructure considered.

Human experts scoring sleep according to the American Academy of Sleep Medicine (AASM) rules are forced select, for every 30-second epoch, one out five stages, even if characteristics neurological signals ambiguous, a very common occurrence in clinical studies. Moreover, cannot score studies where these have not been recorded, such as home apnea testing (HSAT). In this topic review we describe how artificial intelligence can provide consistent and reliable stages based on recorded polysomnography (PSG) cardiorespiratory HSAT. We also show estimates stage probabilities, usually displayed hypnodensity graph, be used quantify ambiguity stability. As an example application characterization disordered breathing (SDB), compared 49 patients with healthy controls revealed severity-depending increase decrease stability during non-rapid eye movement (NREM) sleep. using autoscoring signals, HSAT-derived apnea-hypopnea index hypoxic burden well correlated PSG indices 80 patients, showing technology truly enable HSATs alternatives diagnose SDB.

Sleep-related painful erection (SRPE) is a parasomnia defined by the repetition of erections during rapid eye movement (REM) sleep. Hypnic headache (HH) primary occurring exclusively at night, often REM We report observation 33-year-old man with simultaneous SRPE and HH. Physical examination was normal.
