Abstract The gut microbiota plays a critical role in maintaining human health, influencing wide range of physiological processes, including immune regulation, metabolism, and neurological function. Recent studies have shown that imbalances composition can contribute to the onset progression various diseases, such as metabolic disorders (e.g., obesity diabetes) neurodegenerative conditions Alzheimer's Parkinson's). These are often accompanied by chronic inflammation dysregulated responses, which closely linked specific forms cell death, pyroptosis ferroptosis. Pathogenic bacteria trigger these death pathways through toxin release, while probiotics been found mitigate effects modulating responses. Despite insights, precise mechanisms influences diseases remain insufficiently understood. This review consolidates recent findings on impact immune‐mediated inflammation‐associated conditions. It also identifies gaps current research explores potential advanced technologies, organ‐on‐chip models microbiome–gut–organ axis, for deepening our understanding. Emerging tools, single‐bacterium omics spatial metabolomics, discussed their promise elucidating microbiota's disease development.

Radiotherapy (RT) is one of the major cornerstones in managing gastrointestinal (GI) cancers. However, several side effects, such as intestinal inflammation, mucosal injury, and dysbiosis, often compromise this. The gut microbiota increasingly attracts much interest an essential modulator RT effects influencing immune responses tissue repair. Through short-chain fatty acids butyrate, representatives certain bacterial species play a crucial role under normal conditions, keeping integrity intact reducing oxidative stress-mediated damage. Dysbiosis, state where diminished microbial diversity increased pathogenic are seen, amplifies RT-induced toxicity patients. Clinical investigations highlight that microbiota-targeted interventions, including probiotics, prebiotics, fecal transplantation, hold means to augment efficacy lessen toxicity. Increased microflora specific profiles have yielded serious patient improvements. Advanced methods use stereotactic body radiotherapy combined with modulation promising technique shield healthy maximize immune-mediated antitumor effects. Additionally, there implication tumor behavior regulated by intratumoral regarding response radiotherapy. Notably, provides avenue optimize benefits GI cancers, underscoring importance personalized therapy.

Colorectal cancer (CRC) remains a leading cause of cancer-related deaths worldwide, with its progression intricately linked to gut microbiota dysbiosis. Disruptions in microbial homeostasis contribute tumor initiation, immune suppression, and inflammation, establishing the as key therapeutic target. Fecal transplantation (FMT) has emerged transformative approach restore balance, enhance responses, reshape microenvironment. This review explores mechanisms underlying FMT’s potential, evaluates advantages over other microbiota-based interventions, addresses challenges such donor selection, safety concerns, treatment standardization. Looking forward, integration FMT into personalized CRC therapies requires robust clinical trials identification predictive biomarkers optimize efficacy safety.

The gut microbiome, a complex community of microorganisms residing in the intestinal tract, plays dual role colorectal cancer (CRC) development, acting both as contributing risk factor and protective element. This review explores mechanisms by which microbiota contribute to CRC, emphasizing inflammation, oxidative stress, immune evasion, production genotoxins microbial metabolites. Fusobacterium nucleatum, Escherichia coli (pks+), Bacteroides fragilis promote tumorigenesis inducing chronic generating reactive oxygen species, producing virulence factors that damage host DNA. These can also evade antitumor response suppressing cytotoxic T cell activity increasing regulatory populations. Additionally, microbial-derived metabolites such secondary bile acids trimethylamine-N-oxide (TMAO) have been linked carcinogenic processes. Conversely, microbiota, including Lactobacillus, Bifidobacterium, Faecalibacterium prausnitzii, homeostasis short-chain fatty (SCFAs) like butyrate, exhibit anti-inflammatory anti-carcinogenic properties. beneficial microbes enhance barrier integrity, modulate responses, inhibit tumor proliferation. Understanding dynamic interplay between pathogenic is essential for developing microbiome-based interventions, probiotics, prebiotics, fecal transplantation, prevent or treat CRC. Future research should focus on identifying biomarkers early CRC detection exploring personalized microbiome-targeted therapies. A deeper understanding host–microbiota interactions may lead innovative strategies management improved patient outcomes.
