Disruption of the molecular pathways during physiological wound healing can lead to raised scar formation, characterized by rigid, thick tissue with associated symptoms pain and pruritus. A key mechanical factor in development is excessive tension at site. Recently, microneedles (MNs) have emerged as promising tools for management they engage provide them off-loading from both internal external sources. This review explores mechanisms which physical intervention drug-free MNs alleviates on fibroblasts within tissue, thereby promoting remodeling reducing severity. Additionally, role an efficient cargo delivery system controlled sustained release a wide range therapeutic agents into highlighted. Abstract Hypertrophic scars (HTSs) are the result of an abnormal healing process resulting from burns and other severe traumas. The symptoms that condition include skin irritation, discomfort, itching. This study aimed to assess efficacy fractional carbon dioxide (CO 2 ) laser therapy alone or with triamcinolone 5-fluorouracil (FU) in treatment early post-burn hypertrophic develop during first 6 months after injury. A prospective, randomized, single-blind comparative was conducted on 30 patients aged 16–65 (HTS) burns. Patients had no prior for their scars. We randomly assigned participants one three groups: Group received CO alone, B topical 5-fluorouracil, C acetonide. All groups showed significant improvements ( p < 0.05) overall scar severity height. (fractional + triamcinolone) demonstrated pliability, height, pigmentation 0.05). In contrast, 5-FU) reductions vascularity, height following While all reported minor changes pain itching, there were differences these between C. HTSs this trial revealed surface area thickness improvement pliability pigmentation; however, not statistically difference effect acetonide (TAC), suggesting neither drug offers better over other. Level I, singleblinded randomized control study.

Spalt-like transcription factor 4 (SALL4) plays a vital role in the progression of many human diseases. However, and mechanism SALL4 regulates keloid formation remain unclear. The mRNA levels ubiquitin-specific peptidase 37 (USP37) tissues fibroblasts were determined by quantitative real-time PCR. Western blot was performed to measure protein SALL4, USP4/10/37, collagen-related markers, PI3K/AKT-related markers. growth, invasion migration using CCK8 assay, EdU flow cytometry, transwell assay wound healing assay. Cell glycolysis assessed detecting glucose consumption lactate production. interaction between USP37 confirmed co-immunoprecipitation had increased expression fibroblasts. Silencing inhibited invasion, migration, extracellular matrix (ECM) accumulate fibroblast, while its overexpression opposite effects. In terms mechanism, stabilized through deubiquitinating. Functional experiments suggested that reversed inhibitory effect knockdown on fibroblast functions. Moreover, USP37/SALL4 axis could increase activity PI3K/AKT pathway, PI3K pathway inhibitor LY294002 abolished SALL4-mediated promoting USP37-activated might enhance ECM accumulation via activating pathway.

Abstract Galectin‐1, a constituent of the mammalian β‐galactoside‐binding lectin family, plays pivotal regulatory role in fibrotic cascades. Dysregulated fibrogenic cellular activity has been implicated as critical driver hypertrophic scar (HS) pathogenesis. Nevertheless, precise mechanistic contributions and molecular pathways through which Galectin‐1 modulates HS development remain incompletely characterized. qRT‐PCR western blot techniques were employed to explore expression tissues cells. The knockdown cell line was established by utilizing lentivirus approach, influences on biological functions examined. mechanism underlying regulation investigated via RNA‐seq analysis, immunofluorescence, Western blot. Hypertrophic scars are a common fibrotic skin disorder with high recurrence rate. While various treatment options exist, their efficacy remains uncertain. Manual fractional technology CO2 laser technology, novel approach, has shown promise; however, its effectiveness as standalone ablative is limited. This retrospective study aims to evaluate the clinical outcomes of combining triamcinolone acetonide and 5-fluorouracil transdermal delivery for hypertrophic scars. retrospectively evaluated MFT combined in treating A cohort 42 patients 48 underwent combination therapy thrice. Scar evaluations were conducted using patient observer scar assessment scale (POSAS) before one month after each session. Adverse complication reactions monitored, followed 6 months post-treatment. satisfaction assessed. The total POSAS score significantly decreased from pre-treatment [48.00 (43.00, 53.75) VS 21.29±7.167; p<0.001]. In subgroup analysis, varied by type, quantity genetic factors influencing remission time. rate during 6-month follow-up was 6.24%, 83.34%. Keloid treatment remains a challenging task due to the high recurrence rates and limited effectiveness of monotherapies. Objective: To determine efficacy combining intralesional Triamcinolone Acetonide (TA) with cryotherapy. Methods: A quasi-experimental study was conducted at Dermatology Department, Nishtar Hospital, Multan, over 6 months from 30th June 2024, December 31, 2024. Eighty patients were enrolled using non-probability consecutive sampling. Baseline characteristics, including keloid dimensions, pain, itching scores, documented. Patients received cryotherapy followed by TA (40 mg/mL) injections for up six sessions. Treatment outcomes assessed two weeks post-treatment through reduction in height, length, Visual Analog Scale (VAS) scores. Results: The mean height length reduced significantly 4.66 ± 1.37 mm 1.69 1.18 6.64 2.23 cm 2.68 1.79 cm, respectively (p < 0.001) post-treatment. VAS pain scores decreased 2.86 1.96, 3.53 1.39 0.001). Efficacy, defined as ≥50% achieved 75% participants. Common adverse effects included blistering (18.8%) erythema (15%). Conclusions: combination triamcinolone acetonide demonstrated effective significant improvement Although varied across demographic lesion-specific variables, safe viable option management, manageable promising outcomes.

Keloids are abnormal cutaneous wound healing responses extending beyond the borders of initial wound, usually appearing pink-purplish to hyperpigmented nodules or plaques with a hard consistency, irregular shape, uneven border, and smooth shiny surface. Most often occur on chest, shoulder, upper arms, earlobes, cheeks. This case report aims compare keloid treated intralesional triamcinolone acetonide (TAC) alone acetonide-5-fluorouracil (TAC + 5-FU) combination injection. A 21-year-old Minahasa male complains growing pruritic scars in back area right left arms since five years ago. Abstract Globally, a great number of children have been suffering from physical dysfunction and psychological stress due to uncontrollable scar growth lack effective modalities. Despite chemotherapy's established role as primary treatment for pathological scarring in adults, its efficacy preventing or minimizing formation paediatric patients remains underexplored. This retrospective cohort study aimed refine the relevant clinical evidence investigate effect chemotherapy on scars children. In this single‐centre study, data aged ≤18 years who underwent thoracic surgery at Children's Hospital Fudan University between 1 January 2018, 31 December 2021 were assessed. The outcome was scarring, secondary outcomes subjective symptoms accompanying such pain itching. To mitigate indication bias, analysis performed by inverse probability weighting (IPTW) log‐binomial regression models. comprised 102 children, among whom 36 received adjuvant perioperatively, while 66 did not. Under IPTW model, statistically significant difference incidence observed non‐chemotherapy groups (16.7% vs. 29.4%, p = 0.027). And post‐operatively had lower relative risk compared with those both before after (19.8% 28.8%). Adjuvant may reduce post‐operative
