Disruptions in health service delivery and utilization during the COVID-19 pandemic may have caused many children worldwide to not receive vital preventative services. We investigate pandemic's effects on routine childhood vaccinations India, which has world's largest child immunization program. Using data from Government of India's management information system interrupted time series analyses, we estimate district-level changes relative typical monthly pre-pandemic period. Our results indicate there were significant reductions pandemic, with declines being extremely large April 2020 when a strict national lockdown was place. For example, administration final required dose polio declined by about 60% vaccination levels observed prior pandemic. Vaccinations subsequently increased but largely remained below before outbreak COVID-19. Additional occurred 2021 second wave infections India. Heterogeneity analyses suggest that most districts strictest lockdowns low capacity at baseline.
