Patients with sickle cell disease were presumed to be at high risk for severe COVID-19 outcomes due their compromised immunity and chronic comorbidities. This study aimed evaluate vaccination patterns, healthcare utilization, clinical in a cohort of patients during the pandemic Rio de Janeiro. A total 289 over 18-year-old from Epidemiology Donor Evaluation Study (REDS-III) Brazil followed between January 2021 August 2023. Sociodemographic data, emergency department visits, hospitalizations, mortality rates, status collected. SARS-CoV-2 infection was confirmed by reverse transcription polymerase chain reaction testing symptomatic or hospitalized patients. Of participants, 89.2% completed primary schedule, 62.2% received first booster, 30% second 4.1% all five doses. Emergency visits increased slightly but primarily vaso-occlusive crises. 119 tested SARS-CoV-2, six positive, presenting mild symptoms no COVID-19-related deaths. Vaccination rates similar those general population, Oxford/AstraZeneca Pfizer being most used vaccines. The findings suggest that not significant trigger crises outcomes. High adherence likely played key role preventing COVID-19, alongside other factors such as social isolation herd immunity. However, overlap may have caused diagnostic challenges. Importantly, low morbidity observed emphasize protective effect vaccines, despite presence thromboplastic activity pro-inflammatory states inherent disease. Addressing vaccine hesitancy remains crucial, particularly booster doses show declining adherence. had limited impact on this cohort, triggering potential environmental biological contributed effect.
