Abstract This review compiles information from the literature on chemical composition, pharmacological effects, and molecular mechanisms of earthworm extract (EE) suggests possibilities for clinical translation EE. We also consider future trends concerns in this domain. summarize bioactive components EE, including G‐90, lysenin, lumbrokinase, antimicrobial peptides, serine protease (ESP), polyphenols, detail antitumor, antithrombotic, antiviral, antibacterial, anti‐inflammatory, analgesic, antioxidant, wound‐healing, antifibrotic, hypoglycemic activities action EE based existing vitro vivo studies. further propose potential anticancer lipid‐modifying therapies, its promise as source a novel agent wound healing resistance to antibiotic tolerance. The enzyme lumbrokinase embodies highly effective anticoagulant thrombolytic properties has advantage not causing bleeding phenomena due hyperfibrinolysis. Its antifibrotic can reduce excessive accumulation extracellular matrix. glycolipoprotein G‐90 effectively scavenge reactive oxygen groups protect cellular tissues oxidative damage. Earthworms have evolved well‐developed defense mechanism fight against microbial infections, agents shown good fungal, viral experiments alleviate inflammatory responses caused by reducing pain. Recent studies highlighted role lowering blood glucose. shows high medicinal value is expected be many compounds.
