Abstract Objective To provide a new insight into the diagnosis and treatment of hemiplegic shoulder pain (HSP) by investigating changes in serum mediators. Design Cross‐sectional study. Subjects/patients Shoulder group ( n = 34) control 21). Methods Pain‐free mobility, anxiety status, depression were measured passive range motion (PROM), self‐rating scale, scale (SDS), visual analog respectively. The enzyme‐linked immunosorbent assay was used to test mediators, including interleukin (IL)‐1β, IL‐2, IL‐6, IL‐10, nerve growth factor (NGF), tumor necrosis factor‐α (TNF‐α), substance P (SP), calcitonin gene‐related peptide (CGRP), bradykinin (BK), 5‐hydroxytryptamine (5‐HT), prostaglandin E2 (PGE2), lysophosphatidic acid (LPA). Results pain‐free PROM significantly lower than p < .01), SDS index score higher .05). rate spasticity flexor elbow muscles is .01). CGRP, IL‐2 upregulated compared with whereas NGF, TNF‐α, 5‐HT, PGE2, SP, LPA, BK, IL‐1β decreased Conclusion Patients HSP have risk joint mobility disorders depression; may be an important development pain; CGRP thought major mediator HSP, not inflammatory.

Capsaicin (CAP) induces transient pain sensation by activating receptor potential vanilloid-1 (TRPV1). However, the initial neuronal excitation induced CAP is followed a prolonged refractory period, resulting in long-lasting analgesia. Although effects of on microglia dorsal root ganglion neuropathic disorders have been reported, regulatory pathways remain poorly defined. A chronic model was established via plantar injection complete Freund's adjuvant (CFA), and different doses were administered to rats. Pain behavior, expression pain-related factors, protein TRPV1 nerve cells, inflammatory activation evaluated. In vitro experiments conducted explore migration ability microglia, cytokines pathway proteins, intracellular calcium concentration under CAP.
