Background Neoadjuvant chemoimmunotherapy (nCIT) has shown promise in treating early-stage triple-negative breast cancer (eTNBC), but predictive biomarkers for pathological response and prognosis remain poorly defined. Objective This study aimed to explore complete prognostic factors eTNBC patients treated with nCIT. Materials methods We retrospectively analyzed 112 who underwent surgery after nCIT at Sun Yat-sen University Cancer Center between June 2019 2023. Pathological was assessed using Miller-Payne grade. Clinicopathological features hematologic markers were univariate multivariate logistic regression or Cox regression, as well Kaplan-Meier survival curves. rate (ORR), (pCR), disease-free (DFS) evaluated. Nomograms predicting pCR DFS constructed based on significant risk the systemic inflammatory index (SIRI). Results Higher baseline lymphocyte counts ( P =0.004) independently associated a higher rate, while elevated monocyte =0.006), neutrophil-to-lymphocyte ratio =0.005), platelet-to-lymphocyte (p = 0.005), SIRI =0.037), immune-inflammation =0.029), preoperative =0.010) lower rate. 0.009) correlated shorter DFS, =0.019) predicted longer DFS. incorporating showed high accuracy Conclusion Hematologic markers, particularly SIRI, are cost-effective reliable predictors of treatment efficacy undergoing nCIT, helping clinicians develop personalized strategies.
