Abstract The UHPLC-MS/MS analytical method was used to assess the occurrence of 52 pharmaceutical substances in surface water samples from Czech Republic and Slovakia. Of 29 analyzed, 28 contained at least one quantifiable analyte. This study represents a comprehensive comparative analysis pharmaceuticals waters these neighbouring countries reveals remarkable regional differences level contamination compounds, which depend mainly on population density, altitude flow. Median concentrations key analytes included 126.35 ng dm− 3 for caffeine, 50.81 diclofenac, 134.22 gabapentin, 177.57 iomeprol. findings contribute new knowledge transboundary environmental burden support efforts reduce potential health risks.
