Abstract This study describes the process of developing a high‐impact, low‐cost, and low‐maintenance air ventilation system for anatomy facilities. It employed strategic application Value Engineering (VE), assuring that meets contemporary threshold limit values (TLVs) formaldehyde in working zone dissection tables. A creative–innovative construction methodology was used, combining Theory Inventive Problem Solving (TRIZ/TIPS) VE an laboratory concept. Purpose Publicly funded healthcare systems struggle to govern and determine how finite resources should be allocated in relation political goals within a pre-determined budget. Primary (PHC) has central multipurpose function, not least terms of strategies, but PHC governance is still largely underexplored. The aim explore politicians responsible for making decisions pertaining coverage navigate the public disentangle it form narratives based on different types underlying rationales. Design/methodology/approach Semi-structured interviews were conducted with 15 from 3 Swedish regional authorities. data analysed abductively scientific, clinical cultural rationales using thematic content analysis. Findings Our study provides insights into PHC’s function implicates tensions between politicians' responsibilities regarding coverage. It shows various coexisting rationales, some being more dominant than others where also exist them. In this balancing act, they create addressing stakeholders priority-setting dilemmas, reflecting diverse reveals that play crucial role priorities, might otherwise become overly dominant.
