Purpose of review This explores the use brain age estimation from MRI scans as a biomarker health. With disorders like Alzheimer's and Parkinson's increasing globally, there is an urgent need for early detection tools that can identify at-risk individuals before cognitive symptoms emerge. Brain offers noninvasive, quantitative measure neurobiological ageing, with applications in diagnosis, disease monitoring, personalized medicine. Recent findings Studies show Alzheimer's, mild impairment (MCI), have older ages than their chronological age. Longitudinal research indicates brain-predicted difference (brain-PAD) rises progression often precedes decline. Advances deep learning multimodal imaging improved accuracy interpretability predictions. Moreover, socioeconomic disparities environmental factors significantly affect aging, highlighting inclusive models. Summary promising future risk neurodegenerative disease, monitoring progression, helping prognosis. Challenges implementation standardization, demographic biases, remain. Future should integrate biomarkers to enhance diagnosis intervention strategies.
