Introduction: This systematic review evaluates the application of motion capture analysis (MCA) in assessing postoperative functional outcomes adolescent idiopathic scoliosis (AIS) patients treated with spinal fusion. Material and Methods: A comprehensive search PubMed, Scopus, Embase, Cochrane Library was conducted for studies published between January 2013 September 2024. Eligible included original research examining AIS patients’ post-spinal fusion, specifically kinematic via MCA. Key gait parameters, range (ROM), trunk–pelvic kinematics. Results: Nine comprising 216 participants (81.5% female), predominantly Lenke 1 3 curve types. MCA revealed significant improvements symmetry, stride length, kinematics within one year surgery. Enhanced mediolateral stability normalized transverse plane were commonly observed. However, persistent reductions thoracic–pelvic ROM flexibility highlight limitations. Redistributing mechanical loads to adjacent unfused segments raises concerns about long-term compensatory mechanisms risks segment degeneration. Conclusions: While fusion effectively restores coronal sagittal alignment improves mobility, limitations dynamic adaptability necessitate targeted rehabilitation. Future should standardize methodologies explore motion-preserving surgical techniques address residual deficits.
