Objective: Cognitive impairment is a prevalent complication among patients with diabetes mellitus. It tends to be more prominent in diabetic retinopathy (DR) compared without DR (NDR). However, the functional connectivity (FC) between bilateral cerebral hemispheres both remains poorly understood. This study aimed investigate altered brain and NDR. Subjects Methods: We selected 26 DR, 30 NDR, healthy controls (HCs) participate resting-state magnetic resonance imaging (rs-fMRI) high-resolution T1-weighted structural scans. employed DPABI toolbox MATLAB preprocess acquired images applied voxel-mirrored homotopic (VMHC) FC analysis methods estimate differences 3 groups. The also underwent neuropsychological assessment scales. utilized partial correlation explore associations aberrant connections clinical variables as well characteristics DR. Receiver operating characteristic (ROC) was conducted assess diagnostic performance of VMHC values distinct regions for differentiating from NDR patients. Results: results showed significantly across groups, including lingual gyrus (LING_B), superior temporal (STG_B), postcentral (PoCG_B). Significant were found LING_B, right cuneus (CUN_R), STG_R, PoCG_B, precentral (PreCG_R), precuneus (PCUN_R), middle (MTG_L) Moreover, negative noted LING_B disease duration Positive correlations detected PoCG_B fasting blood glucose (FBG) levels. Furthermore, ROC demonstrated that combining all differential achieved highest area under curve 0.826. Conclusions: alterations may reflect underlying neuropathology cognitive dysfunction These patterns could serve neuroimaging biomarkers, offering insights into early diagnosis intervention impairments
