Background/Objectives: Non-small cell lung cancer (NSCLC) patients without gene driver mutations receive anti-PD1 treatments either as monotherapy or in combination with chemotherapy based on PD-L1 expression tumor tissue. Anti-PD1 antibodies target various immune system components, perturbing the balance between cells and soluble factors. In this study, we identified signatures of NSCLC associated different clinical outcomes through network analysis. Methods: Twenty-seven metastatic were assessed at baseline for levels circulating CD137+ T (total, CD4+, CD8+) via cytofluorimetry, along 14 checkpoints 20 cytokines Luminex Hierarchical clustering connectivity heatmaps executed, analyzing response to therapy (R vs. NR), performance status (PS = 0 PS > 0), overall survival (OS < 3 months OS months). Results: The revealed three groups a significant differential proportion six (p 0.0001). Furthermore, pairwise correlations among factors evaluated R compared lack same NR vice versa. These comparisons conducted months. results indicated that ≤ exhibited an inflammatory-specific signature contrasting conditions characterized by checkpoint molecule-based 0.05). Conclusions: Identifying profiles linked therapy, PS, represents findings can optimize therapeutic choices.
