Diabetic retinopathy (DR) is one of the most prevalent complications diabetes, affecting nearly one-third patients with diabetes mellitus and remaining a leading cause blindness worldwide. Among various diabetes-induced complications, DR particular importance due to its direct impact on vision irreversible damage retina. characterized by multiple pathological processes, primarily hyperglycemia-induced inflammatory response oxidative stress. Current gold standard therapies, such as anti-VEGF injections photocoagulation, have shown efficacy in slowing disease progression. However, challenges drug resistance, partial therapeutic responses, reliance eye injections—which often result low patient compliance—remain unresolved. This review provides comprehensive overview underlying molecular mechanisms DR, current their unmet needs for treatment. Additionally, emerging strategies improving treatment outcomes are discussed.
