The rise of Internet hospitals has significant issues associated with data security and governance in managing sensitive patient data. This paper discusses an alliance blockchain (i.e., a private blockchain) model for innovation internet improved encryption methodology. We compare our proposed model, Rivest–Shamir–Adleman (RSA) encryption, integrated into the framework. Improved RSA achieves impressive improvements all key metrics by increasing throughput 24.7% lowering latency 19.8% compared to base model. The advent of Tiny Machine Learning (TinyML) has unlocked the potential to deploy machine learning models on resource-constrained edge devices, revolutionizing real-time monitoring in Internet Medical Things (IoMT) applications. This study introduces a novel approach electrocardiogram (ECG) anomaly detection by integrating TinyML with Artificial Intelligence (AI) low-power embedded systems. Abstract Healthcare services must fulfill patients’ desires for secure data sharing and high accessibility. Blockchain technology, through blockchain platforms (BPs), can overcome healthcare challenges. This study develops a decision-making methodology selecting the best BP, by integrating with IoT Metaverse, proposed approach ensures integrity, quality, privacy security, sharing, interoperability. The uses multi-criteria (MCDM) to handle conflicting criteria. Two MCDM methods are used in this study: CRiteria Importance Through Intercriteria Correlation (CRITIC) weight computation, Ranking of Alternatives Weights Criterion (RAWEC) alternative ranking. To deal uncertainty, concept spherical fuzzy sets (SFSs) is utilized, RAWEC method extended under SFSs first time. applied case new town Egypt, considering twenty-two criteria fifteen alternatives. results show that performance criterion has highest weight, latency lowest. sensitivity analysis was conducted stability rank.
