Background Brain glymphatic dysfunction may contribute to the development of α-synucleinopathies. Yet, noninvasive imaging and quantification remain lacking. Purpose To examine function brain in isolated rapid eye movement sleep behavior disorder (RBD) its relevance phenoconversion with use diffusion-tensor (DTI) analysis along perivascular space (ALPS). Materials Methods This prospective study included consecutive participants diagnosed RBD, age- sex-matched control participants, Parkinson disease (PD) who were enrolled examined between May 2017 April 2020. All underwent 3.0-T MRI including DTI, susceptibility-weighted susceptibility map–weighted imaging, and/or dopamine transporter using iodine 123–2β-carbomethoxy-3β-(4-iodophenyl)-N-(3-fluoropropyl)-nortropane SPECT at time participation. Noninvasive vivo imaging is expected useful elucidating pathophysiology these Currently, magnetic resonance most commonly used technique evaluate humans, large number studies have reported. This review provides comprehensive overview investigations human using imaging. can divided into 3 categories, including without gadolinium-based contrast agents (GBCAs), with intrathecal administration GBCAs, intravenous GBCAs. purpose examine not only movement brain parenchyma, but also dynamics perivascular subarachnoid spaces, well parasagittal dura meningeal lymphatics. Recent research even extended include eye inner ear. serves an important update guide future directions.

Objectives: Dementia is a clinical syndrome caused by multiple etiologies, usually manifests with progressive and diffuse brain dysfunction. The activity of the human glymphatic system was evaluated in cases dementia diffusion tensor image analysis along perivascular space (DTI-ALPS). Methods: We recruited 28 healthy subjects 77 patients, including 38 Alzheimer’s disease (AD),18 mild cognitive impairment (MCI), normal controls (NC) 21 vascular (VCI). All participants underwent DTI scanning. Diffusivities X, Y Z axes were obtained lateral ventricle body plane all subjects. assessed diffusivity spaces, as well projection fibers association fibers, respectively, order to acquire an DTI-ALPS-index correlated them mini mental state examination (MMSE) montreal assessment (MOCA) scores using partial correlation which influence age controlled. Results: AD, MCI, VCI patients showed significantly lower (p < 0.001) compared NC. Besides, group had higher than AD = 0.007). There significant positive between MMSE MOCA (the effect controlled), showing that water spaces associated dementia. The Dzassoc led reduced VCI, while Dxassoc contributed decrease AD. Conclusion: evaluation DTI-ALPS demonstrates decreased DTI-ALPS-index. Different from show drainage disorder rather impairment. DTI‐ALPS correlations clinical (migraine severity [numeric rating scale]/disability [Migraine Disability Assessment (MIDAS)]/bodily pain [Widespread Pain Index]/sleep quality [Pittsburgh Sleep Quality Index (PSQI)]) examined. Abstract Glymphatic system is an emerging pathway of removing metabolic waste products and toxic solutes from the brain tissue. It made a network perivascular spaces, filled in cerebrospinal interstitial fluid, encompassing penetrating pial vessels communicating with subarachnoid space. separated by blood barrier tissue endfeet astrocytes rich aquaporin 4, membrane protein which controls water flow along Animal models magnetic resonance (MR) studies allowed to characterize glymphatic function determine how its impairment could lead numerous neurological disorders (e.g. Alzheimer’s disease, stroke, sleep disturbances, migraine, idiopathic normal pressure hydrocephalus). This review aims summarize role pathophysiology migraine order provide new ways approaching this disease therapy.
