Aims Reconstruction of soft tissues and preservation the extensor mechanism integrity provide a major challenge following resection proximal tibia tumours. We propose novel surgical technique for neopatellar ligament reconstruction using hamstring tendon in tumour cases. This study details technique, early clinical radiological outcomes, potential long-term benefits this innovative reconstructive approach megaprosthesis implantation. Methods included 15 patients with tumours treated at orthopaedic oncology unit Gujarat Cancer Research Institue (GCRI), Ahmedabad, India. Surgical procedures resecting tumour, implanting megaprosthesis, tendons to reconstruct by ligament. Autologous (gracilis semitendinosus) harvested from ipsilateral leg were used these braided together nonabsorbable suture threaded through remaining patellar tendon. The then secured holes tibial tuberosity implant XLO (Ortho Life Systems, India) fibre wire sutures FiberWire (Arthrex, USA). tension was optimized tightened knee full extension covered medial gastrocnemius muscle flap. mean follow-up period 19 months (13 24), age group 24.6 years (14 44), it 11 males four females. Results histopathological diagnosis osteosarcoma (eight patients), Ewing’s sarcoma (four giant cell (three patients). duration 3.2 hours (2.5 4.2) blood loss 250 ml (150 450).Wound infection found three cases, which managed debridement antibiotics. None had local recurrence latest follow-up. active flexion 92° (85° 105°), an lag 4.6° (0° 10°). patella height preoperatively 4.5 cm (4.3 5.1), length 4.7 5.68). preoperative height-to-patella ratio 0.96 (0.89 1.02). Postoperatively (at 12 months), 4.4 (4.1 height-to- 1.04 (1 1.14). Follow-up examinations showed that integrated satisfactorily, no obvious signs graft rupture or elongation.The Musculoskeletal Tumor Society score 24 (22 27). Conclusion presents Salunke’s Institute (GCRI) successfully restores knee's mechanism. provides effective option preserving flexion, extension, minimizing lag.
