Abstract Background Although fibromyalgia is a disabling disease, there no targeted therapy for specific neurotransmitters or inflammatory mediators. Our aim was to provide neurologists with practical guidance the management of these difficult patients based on critical, narrative and non-systematic review randomized controlled trials (RCTs) from last 10 years. Methods The members Special Interest Group Neuropathic Pain Italian Neurological Society evaluated years answered questions that allow consensus main pharmacological non-pharmacological approaches. Results neuropathic pain working group agreed prescribing antiepileptic drugs antidepressants in case comorbidities anxiety depression. Fibromyalgia represents a chronic pain pathology characterized by severe musculoskeletal pain, fatigue, disturbances in sleep, and cognitive issues. Despite its presence, the underlying mechanisms of fibromyalgia remain inadequately understood; however, recent investigations have suggested that inflammation could play fundamental role pathophysiology this condition. Several studies highlight elevated concentrations pro-inflammatory cytokines, dysregulation immune responses, neuroinflammation patients. Furthermore, low-grade has been proposed as potential catalyst for sensitization pathways, which exacerbates symptoms fibromyalgia. Understanding disease might open new avenues therapeutic interventions while providing more profound insight into complex nature debilitating disorder. Although progress made, further research is needed to uncover complexities involved. This review investigates intricate relationship between fibromyalgia, analyzing evidence supports involvement both peripheral central inflammatory processes onset persistence

The global aging population faces a growing prevalence of sarcopenia and musculoskeletal (MSK) pain, two interrelated conditions that diminish physical function, quality life, independence in older adults. Sarcopenia, characterized by the loss muscle strength, mass, often coexists with MSK emerging evidence suggesting each condition may contribute to progression other. This perspective explores bidirectional relationship between highlighting shared mechanisms, including inactivity, cellular aging, chronic inflammation, gender-related hormonal changes, psychosocial factors such as depression social isolation, which underlie mutual exacerbation conditions.
